Employee lost grip on bin handle, slipped, and fell fracturing right foot.

Miner was installing wheel hub on truck. Miner pinched small finger on left hand while attempting to align valve stem. Required 3 stitches on small finger of left hand.

Employee was standing on ground washing out material buildup when employee turned hitting head on steel structure with forehead. Causing a laceration above nose resulting in three stitches.

Employee was tightening bolts on loader cutting edge. The edge slipped catching employee's watch wrist band causing lacerations to left hand.

Backed up to dump point, raised bed to dump material, ground gave way under back tires, truck upset. Driver complained of back and knee pain possibly caused by seat belt jolt.

Mechanic was repairing hoist cylinder. Attempting to hook cable hoist sheave, slipped away from grasp and hit right foot causing contusion. Went for x-ray (negative).

On 4-11-07 employee pulled on bin gate that was slightly frozen straining right shoulder. Treated with physical therapy with no lost time until worker's compensation decision that surgery was needed. Surgery on 08-16-2007.

On 5-4-07 slipped off step while climbing off water truck. Caught self with right arm to avoid fall, straining shoulder. No lost time as tried therapy to rehab shoulder. Did not respond so Worker's Comp authorized shoulder surgery on 7-23-07. Return to work date not determined at this time.

EE WAS LIFTING PIECE OF STEEL & DROPPED IT, CUTTING HIS RIGHT RING FINGER. EE RECEIVED 6 STITCHES.
