When moving a stone from one pallet to the finished pallet for packaging with a suction cup tool employee reached down to move some packing material and at the same time the stone fell from the suction cup onto EE's hand

Employee was working indoors in the Chilton Manufacturing Facility where he is a splitter operator. He was trimming a piece of stone using a chisel and hammer and missed the stone, striking his left hand.

EE WAS USING A PRY BAR TO UNBLOCK ROCKS ON CONVEYOR BELT. WHEN HE LIFTED UP ON THE PRY BAR HE FELT A TWINGE IN HIS BACK. DIAGNOSIS: BACK PAIN.

Employee was using a pry bar to tip a stone over. the stone fell back onto the employee's finger fractured left middle finger.

EMPLOYEE WAS USING A HAMMER & CHISEL ON A STONE THAT WAS BEING HELD UP BY FORKS OF A SKID STEER. THE STONE CRACKED HORIZONTALLY & FELL ON THE EMPLOYEE'S FOOT. EMPLOYEE HAS BROKEN BONES IN HIS LEFT FOOT.

EE WAS USING A HAMMER TO CHIP A STONE. RIGHT ARM BECAME SORE FROM THE REPETITIVE MOTION. DIAGNOSED WITH RIGHT ARM TENDONITIS. EE DID NOT SEEK MEDICAL TREATMENT UNTIL 6/8/04.

EE WAS CHISLING A STONE. STONE CHIP CUT FINGER. RESULT CUT MIDDLE FINGER ON RIGHT HAND - STITCHES WERE NEEDED.

EMPLOYEE CHISELED STONE WITH HAMMER AND CHISEL ON 11/15/03. FELT EYE IRRITATION ON 11/16/03. WAS TREATED IN EMERGENCY ROOM. DOCTOR REMOVED A PIECE OF RUST FROM RIGHT EYE.

EMPLOYEE WAS MOVING A STONE FROM TABLE TO SAW TO BE CUT. ANOTHER EMPLOYEE WHO SHARES THE SAME TABLE WAS USING A CHISEL & HAMMER. THE HAMMER SLIPPED OFF THE CHISEL & HIT THE EMPLOYEE'S LEFT IND EX FINGER. HE RECEIVED A SEVERE CONTUSION.

EMPLOYEE WAS USING A HAMMER TO OPEN A STONE. HE FELT A PAIN IN HIS RIGHT SHOULDER BLADE. DIAGNOSED WITH A ROTATOR CUFF TEAR.

EMPLOYEE WAS CHISELING STONES WITH A CHISEL AND HAMMER WHEN HE BEGAN EXPERIENCING PAIN IN HIS HIP AND LOWER BACK. DIAGNOSIS: LOWER BACK STRAIN.

THE EMPLOYEE WAS OPENING A STONE WITH A HAMMER & TRACER. WHEN THE STONE BROKE, HALF OF IT FELL OFF THE TABLE ONTO HIS FOOT, FRACTURING IT.

EMPLOYEE WAS CHISELING A STONE WHEN A PIECE OF THE STONE BROKE OFF & ENTERED HIS LEG. DIAGNOSIS LACERATION TO LEG, STITCHES NEEDED.

EE WAS TRIMMING STONE WITH A HAMMER AND CHISEL. ARM BECAME SORE. TENDONITIS OF THE RIGHT ARM.

EE WAS CHISELING STONE WITH A HAMMER & CHISEL. A PIECE OF THE CHISEL BROKE OFF & ENTERED HIS LT.FOREARM. E.R. WAS UNABLE TO REMOVE FOREIGN BODY & REFERRED HIM TO HIS PRIMARY PHYSICIAN. HIS DR. REFERRED HIM TO A SPECIALIST. SPECIALIST DECIDED TO LEAVE FOREIGN BODY INTACT.

EE WAS STANDING NEXT TO A CONVEYOR BELT CHISELING OPEN A STONE. A STONE ON THE CONVEYOR BELT BROKE AND A PIECE OF BROKEN STONE FELL ON HIS LEFT HEEL, LACERATING IT.

EE WAS WORKING IN THE SMALL SAW SHOP AND SAID HIS RT. WRIST WAS BOTHERING HIM. THE CONTINUOUS LIFTING AND PULLING DOWN ON THE SAW HURT HIS WRIST. DIAGNOSED AS RIGHT WRIST STRAIN.

THE EE WAS CHISELING STONE, WHEN THE HAMMER GLANCED OFF THE CHISEL AND STRUCK HIS FINGER. THE EE DID NOT SEEK TREATMENT AT THAT TIME. ON 10/21/02 WE RECEIVED A BILL FROM REGIONAL CLINIC. THE CORRESPONDENCE INDICATED THAT THE EE HAD BROKEN THE 4TH FINGER ON HIS LEFT HAND.

WHILE CHISELING STONE AND OPERATING HIS SAW HE STRAINED HIS WRIST.
