Employee works at the Oakfield Quarry as a loader operator. While banding a pallet in the quarry, the employee cut the right thumb and index finger. The banding did not spool as freely as the employee was expecting and when they pulled harder on the banding, it slipped through their hand cutting it.

Employee works outdoors in an above ground quarry operating a loader. While banding a water pump hose, the banding cut EE's hand. Employee sought treatment for the laceration.

Employee was opening a stone; he lost his grip on the stone. The stone fell onto the one behind it pinching the employee's left thumb. Restricted duty assigned.

On 4/22/10, employee was lifting 60-70 lb stones off pallet; on 4/23/10 employee helped pack 100 holes for blasting. Employee's back hurt over the weekend. Employee did not report injury until 4/26/10. He did not seek treatment until 4/30/10. Restricted duty assigned.
