The employee was attaching rope to the miner's boom when the miner shifted, pinching his knee between the boom and the rib. The employee began missing work on 1/9/2014.
The employee was installing a cable bolts. He accidentally hit the rotation button, instead of feed, and the bolt hit him.
The employee pinched his hand between the long arm and the canopy post.
The employee was installing a cable bolts when he experienced pain in his groin. The employee began missing work on 8/27/2012.
The employee was hit in the face with a drill steel while roof bolting. The incident resulted in a fractured jawbone.
The employee was using a grinder when the hand tool kicked back, striking him in his mouth. The incident resulted in a cut lip and a chipped tooth. Stitches were required to repair the laceration to the lip.
He was servicing the miner. The miner bits got caught on the line curtain, so he shut the miner off and tried to get the curtain loose. Since he could not free it, he started the miner to drop the head. He placed his left hand on a bit lug. Instead of lowering the head, he mistakenly raised the head, smashing his left hand between the roof and the miner head.
The employee stated that he was leaning over the scoop batteries to let the bolter cable down. The roof bolter operator was tramming the bolter around the turn, and the pad of the ATRS roof support struck him, pinning him between the scoop batteries and the ATRS.
The bolter operator was prying down loose rock from the top. Some of that rock landed on the canopy of the bolter. When the canopy was moved, the rock fell from the machine and hit the injured employee in the middle of the back.
The employee stated that he was installing a roof bolt and experienced a sharp pain in his stomach. Note: The employee did not begin missing work for this injury until 2/23/2011.
The employee stated that a roof bolt plate sliced through his boot, cutting his right foot. Stitches were required for treatment.
Was cleaning loose material from around cement pump feed area when the auger caught his glove.
The employee stated that he was taking a bolt out of a ride. The impact wrench slipped and cut his finger. Stitches were required.
The employee stated that while roof bolting, he hit his knee on the roof bolt plate, causing the laceration. Stitches were required to close the injury.
The injured was removing steel out the bolter head. A piece of the steel broke off and cut the employee's right forearm.
The injured was pushing raw coal to the plant's raw coal feed bin. He drove too close to the edge in front of him. As he began to back up, the loose coal under the dozer tracks gave way, causing the machine to nose dive. The blade struck the hard packed ground, launching the injured forward into the windshield area and resulting in the injury.
The injured was roof bolting. The bolt broke and a three ton 'come-a-long' fell, smashing the injured's finger.
The employee was standing on the top of the heavy media cyclone pump overflow box. During this time a fellow employee was tramming the overhead hoist across the plant. The frame of the overhead crane struck the victim, wedging him against a six inch plastic pipe. The six inch plastic pipe broke from its restraint, causing the victim to fall to the floor below.
Injured was picking bottom and rib out under the continuous miner to free trailing cable. A small piece of slate flew from under the miner, becoming lodged in the injured's right hand (near index finger). Please note: the injury occurred on May 15, 2009, but the injured did not begin missing work until July 7, 2009.
Installing a 16' cable bolt when pushing the bolt up, employee slipped and hit the joy stick, causing the roof bolt to rotate and go up at the same time, striking his face cutting his nose, upper lip and left cheek.
EE bent a roof bolt during installation. The head of the bolt stuck in the chuck of the bolter. The EE was working his drill head up and down to try and free the roof bolt. He accidentally hit the rotation lever, causing bolt to spin and strike his arm.
The bolter wrench was stuck in the head. Injured was using channel locks to remove the stuck wrench. While jerking on the channel locks, injured twisted his wrist. He continued to bolt for the remainder of the shift. (Bolting helper-side Fletcher RRII). He went to the hospital later for treatment after wrist began swelling and pain increased.
Machine came to sudden stop - cut employee's right arm.
Employee was cutting a beam, the beam rolled off of the scoop and landed on his right foot. Breaking bone behind right big toe.
Injured was operating Fletcher Twin Boom Bolter. Right knee was too close the the bolter drill head. As the head was lowered, it hit right knee causing bruising and strained ligaments in right knee.
Injured was operating a roof bolter, had bent drill steel in hole in roof, while lowering head and removing drill steel, he hit the rotation button steel spun around and hit him in the nose.
Was roof bolting on Fletcher Twin Boom Roof Bolter. Was pushing roof bolt into bolt hole. Bolt was partially in hole & started to bend. While pressure still applied, injured tried to spin bolt to stir resin and bolt bent further and then bent part of bolt hit injured in left wrist.
