Employee was working in the finish mill building cleaning out a blockage in FM1 step chute for approximately 10 hours using a jackhammer. Jackhammer caused dust to stir from the material they were working on. The employee had cleaned self off several times and changed clothes twice throughout the day. Employee noticed the burn the next am before work.

Employee was using a 40 lb electric jackhammer to break up material in the cooler. EE bent over to move some material, and as EE stood up, EE felt a pain in lower back. Employee reported incident to supervisor, was taken to OMP for medical evaluation, placed on restricted duty( no lift push pull>15 lbs, no jackhammering until re-evaluation.

EE was welding in the reject bin when a piece of slag fell behind EE's safety glasses and landed on the inner corner of EE's left eye resulting in a first degree burn. Antibiotics and eye drops were prescribed. No lost time.

The supervisor went to see the pump area were one diesel pump was having problems. At the moment he was getting near to the pump, the valve at the exit of the pump got loose from the fitting and started spray diesel all over the supervisor.

During the third shift the lab technician informs to the plant foreman that she was not feeling well, due a strong smell in the laboratory caused by release of air and oil from the x rays machine compressor in the Lab. The plant foreman reaches Laboratory and felt the strong odor, employee was retired from the area, and compressor was shut off

The employee was sucking out the dust collector at Tower 2. The employee had placed the hose inside the hopper. The hose became clogged with a large chunk of clinker. The employee brought the hose part way out of the hopper to remove the chunk. The employee tried to remove the chunk from the end of the hose, and lost control of the chunk. His hand went into the rotary valve.

Employee was standing in the area around a ball mill when the mill started, dust fell from above, getting behind employees safety glasses and in his eye. Employee went to a nearby eyewash station, rinsed his eyes out thoroughly, and was sent to a Dr. for evaluation. There he received prescription antibiotic eye drops.

Employee was air lancing next to a bulk truck loading snorkle and material blew back up into face and got under safety glasses. Employee was not wearing the proper PPE for the job task. Goggle or face shield are required.

Employee was using a jack hammer at tower 2 near the transition between 31 and 105 belt. The jackhammer blade got stuck in some wire mesh. When employee went to pull it free, the 2" spade landed on his left foot.

Employee was using a hammer drill to drill refractory. Drill caught and spun out of operators hand striking him in the cheek causing a small laceration. Employee received stitches. Laceration occurred at the end of shift, employee returned next day to work.

Accident occurred while demolishing a retired/abandoned building structure in an area isolated from the rest of the plant at shut-down surface area. Excavator slid laterally on concrete floor surface sliding onto lower level area. Excavator landed on its side. Operator only involved in accident and sustained fractured right arm and broken ribs.

Employee was in the process of cleaning out the step chute on FM1 from the 014 East/West belt. The employee looked through the inspection door when the blaster that was installed to keep the chute clear discharged throwing material in his face. Even though he was wearing his safety glasses some material entered his left eye.

The back of the injured employee's hand was cut when the cutting wheel on the hand grinder he was using to cut a slot in a pipe caught and kicked back and struck his opposite hand. Injuries included skin lacerations on the back of his hand.

Employee was cleaning hard build up from the bottom of #1 Kiln Clinker elevator with small chipping hammer. Employee was wearing approved safety glasses, and dust mask. When he activated the handle on the chipper the 5/8" air hose came loose from the hose barb on the quick coupling, and struck the employee in the right eye.

Employee was welding 2D cement line at Silo 59. Employee was holding welding hood in his left hand (hard hat did not have clips to attach hood) and started welding with his right hand. He was having trouble getting the weld to start because pipe was dirty. While trying to get the weld to start the employee flashed himself at least 2 times.

Employee was packing cement and one of the packing spouts activated on its own causing a stream of cement to hit the employee and dust entered the employee's right eye. Employee's eye is red and irritated. Employee was treated and issued eye ointment and patched.

Employee was using an air powered impact wrench and a universal joint and a 1 5/16" socket. While reaching to remove the bolt his finger energized the trigger of the wrench and the socket flew off the universal joint striking employee in the center of his upper lip.

Employee was standing beside the electric water pump while it was running when the 2" ball valve came apart on top of the pump. He was sprayed with water and then the cam lock end of the discharge hose came apart and hit him in the nose. The cam lock struck him in the safety glasses and nose. He received a cut on each side of his nose and his nose was broken.

EE was attempting to grind a burr off of the end of a 1/4" x 1" x 8" piece of metal. When the EE went to feed the material into the floor grinder media, metal kicked & left hand was turned & pulled into grinder media lacerating left thumb. He immediately let go & pulled hand away. He was taken to hospital where they amputated thumb to 1st knuckle.

Supervisor was initiating the start-up process of the Roller Mill,by overload of material in the mill table,he proceeded to rotate the table with the auxiliary motor. The auxiliary motor has a manual coupling to start-up the engine in local; the handle had a pneumatic coupling mechanism; it failed to connect properly causing the handle to bounce back hitting his right hand.

Employee operating the automatic bag filling machine and noticed malfunction of the bag applicator; he removed the guard that protects the pulley and introduced his hand to check the belts; his arm obstructed the photocell device activating the spout mechanism. His left hand got caught between the belt & pulley.

The injured was using a grinder to grind off a pin on a cyclinder. Approximately two days later, 1-8-09, the injured experienced discomfort in his left eye. The injured sought medical attention and had a piece of metal removed from his eye.

While welding, a hot piece of slag burnt through EE's glove and burnt his hand. He took his glove off and the skin where he was burnt came off. The burn is on his right hand on his pinky knuckle and his ring finger knuckle and is about 1" long and 1/2" wide. He went to the first aid room and cleaned the burn, put on antiseptic, and dressed the burn.

Employee was pulling old motor leads from #1 ID fan drive - cutting the wires off with a Sawzall. I was holding the large wires (3/4" copper stranded) in my left hand and running the saw with my right hand. I was bending the wire with my left hand when the saw drew my hand in. I was wearing my cotton work gloves.

Clinker Elevator for #2 Kiln would not run. Found out the Chute going to cross apron was plugged up with dust and large chunks of clinker. Two co-workers and I were trying to get the chute unplugged using a Jack Hammer. While I was using the Jack Hammer inside of chute the material broke loose and pinched my finger between elevator housing and the jack hammer.

EE was rotating a kiln using a ponymotor, 2 clutches are to be engaged. The 1st was engaged, EE was working at 2nd at the time of the accident. As the clutch was engaged, the pony began to labor and in order to avoid stalling, EE began to disengage the clutch. As he disengage it was backspinning driving the clutch plate in reverse, causing failure of the clutch.

He was cutting balls out of the discharge screens in #1 Raw Mill with the plasma cutter. A hot piece of slag rolled down into his glove. causing a burn to rt palm below the thumb. On 4/22 was sent to the dr., put on restrictions and given an antibiotic for his infection, which resulted in an allergic reaction, and he was sent back to the dr. on 4/29.

Employee was welding and suffered welder's flash. Other employees welding behind him caused a flash from their side as well. Employee wearing welding hood which was not automatic and did not drop fast enough. Did not know he was flashed until during the night and reported incident at start of shift the following day.

Contractor was welding on the #1 Kiln Shell. Employee was performing his normal duties, causing him to walk on the catwalk beside the kiln. A small piece of hot slag, approximately the size of a pellet, rolled around the kiln shell, was deflected off the pier and landed on the back of his neck. This caused 2nd degree burns to the back of his neck.

Employee was working in the removal of gear box from the spare drive shaft of E01 chain. Two hydraulic jacks with supplemental pieces were used. While the jacks were in pressure the gear box was heated to clear the interference between the couplings. As the coupling came loose one of the supplements slipped out and hit the employee on his right leg.

Employee was using a cutting torch overhead, holding the torch with right hand. A slag of liquid metal fell and burned through his coat sleeve, and onto the top of his left lower forearm/wrist area. The metal stuck to the employee's wrist watch and melted his watch. Employee received severe (3rd degree) burn to his arm.
