Employee was removing bolts on the counterweight on the excavator. When EE removed the last bolt, the counterweight fell (hydraulic ram bracket failed). The counterweight struck the ladder causing the employee to fall & hit their head. EE was taken by ambulance & was diagnosed with a fractured skull.

Showing all 21Employee was removing nut from bolt using cordless impact wrench on bolt & holding nut with box wrench. Bolt, nut & box wrench twisted causing EE's finger to get caught between box wrench & frame of equipment. EE sought treatment & pin was placed in fingertip for fracture.
Employee was welding on 3/15/16 & hot slag fell down sleeve causing a burn. On 3/23/16, our employee notified us of this incident & sought medical treatment. EE was diagnosed with a burn & an infection.
Employee was grinding on a piece of metal flashing that was resting on the metal saw horses. One side of the flashing vibrated off of the saw horse causing the flashing to slide sideways off of the other saw horse. The flashing cut the employees finger as it was falling. The employee sought medical treatment and received stitches.
Employee was fixing a metal guard on a conveyor. He was using one hand to hold the piece of metal while using the other to hold the grinder. He started to grind and the grinder shifted and contacted his finger. He sought medical treatment and received stitches.
Employee was welding & received burns to both eyes. He did have a welding shield on. He sought treatment & received eye cream & oral medication.
EE was blowing off equipment with air hose. He tried to move the air hose to his other hand and he lost control of it. The air hose struck his safety glasses, knocking them off and then hit his eye. He was given prescription eye drops.
Employee was grinding, welding and cutting on various pieces of equipment throughout the day while wearing eye protection. At end of day, employee's eye was irritated. He sought medical treatment and eye MD removed piece of embedded metal from right eye.
Employee was pressure washing screen deck & debris flew back into eye (left). Sought medical treatment on 6/18/10 & MD removed metal from left eye & prescribed eye drops to prevent infection.
