Over-exertion in lifting objects

Employee was sampling 5"" select larger stones which involves shoveling, carrying & loading buckets to a tailgate. on 05/09/2025 we were notified the Employee sought treatment and was prescribed PT. Reporting within 10 days.

Struck by... (Not Elsewhere Classified)

Employee was lowering the landing gear on a conveyor when the bracket slipped and struck EE's left ring finger. EE sought medical treatment and was diagnosed with a bruise. EE was given a finger splint.

Fall to the walkway or working surface

Employee was walking while moving a conveyor, tripped and fell, rolled EE's left ankle. Employee sought medical treatment, was diagnosed with a sprain. Was taken off work for a couple of days.

Over-exertion in lifting objects

Employee was lifting empty garbage can out of the bed of company vehicle when EE felt pain in the left shoulder. Employee sought treatment on 7/13/23 and was diagnosed with a strain. EE was referred to PT.

Over-exertion in lifting objects

Employee presented with heart attack/stroke symptoms when lifting ice & gravel chunk to remove from conveyor. Due to alleged symptoms, incident was immediately reported to MSHA. Employee sought medical treatment and was taken off work for 1 day for back strain.

Over-exertion (Not Elsewhere Classified)

Employee was clearing off catwalk to mobilize the plant and caught a toe on wood blocking. Employee grabbed the handrails to stabilize self and in doing so strained left shoulder. Employee initially did not seek medical treatment. On 4-10-20, employee sought treatment with a chiropractor.

Fall from machine

Employee was climbing off loader using the attached ladder & while on the last rung employee's foot got caught & employee fell on their side. Employee sought medical treatment & is currently off work.

Struck against stationary object

EE was stepping off ladder & stepped on rock & had pain in right knee. Had MRI don on 9/2/09. We received MRI report on 9/17/09 which states ""possible tear on the posterior horn medical meniscus"". EE continues to work at this point - may have surgery this winter.

Struck by flying object

Employee was grinding belt clip and piece of steel flew into left eye. Employee was wearing safety glasses. Employee went to MD & had steel removed from eye. *Employee did not report injury to us until 3/9/09.

Over-exertion (Not Elsewhere Classified)

EE complained about both hands going numb after he operates the loader. He did not seek medical treatment until 8/17/07. His doctor diagnosed him with CTS. At this point, EE has not lost any time from work. He may have surgery in the future. He has no work restictions at this point. We are still investingating this claim and have not accepted compensability for work related.

Fixing flashing under roller/feedbelt using a prybar. The prybar slipped and struck him in the mouth. Initially he did not seek treatment for this but recently has had dental work for chipped tooth.

Stepping out of skid loader & felt a pull in back. Lumbar back strain.
