EE was performing site maintenance, driving a skid steer, when EE went to exit EE felt pain in EE's lower left back. Employee sought medical treatment on 8/18/25, where EE was ordered to do PT. Reporting within 10 days.

Employee was moving multiple oak hardwood blocks from ground level to a transport crate. We were notified on 7/17/24 that the Employee sought medical treatment and was diagnosed with a hernia.

Employee was lifting a drill steel to insert it into the drill's hammer. As employee was lifting the drill steel EE felt pain in abdomen. EE was diagnosed with a hernia & surgery has not been scheduled yet.

EE was walking on ground at surface mine and had seizure of unknown origin. EE is still in hospital undergoing testing.

Over-exertion in lifting objects

EE was shoveling stone and felt pain in right knee. Initial diagnosis was sprain and he did not receive any restrictions. Pain continued. EE's new diagnosis is a medial meniscus tear. Surgery is on 3/19/12. Note: EE was laid off for end of season after his shift on 12/2/11.

Contact with heat

Employee fainted due to dehydration - fell & cut head. Employee receive 8 stitches in head, was given fluid for dehydration & take off work.

Employee smashed left finger between ratchet & frame while taking a nut off a bolt.

EE was taking bolt out and was using impact wrench. EE put hand/palm underneath to hold bolt and spun belt with wrench. Bolt spun into right hand/palm. EE received stitches.

STEPPING UP ONTO METAL PLATE NEAR CONVEYER WHEELS AND THE PLATE BROKE. EMPLOYEE FELL SCRATCHING AND BRUISING LEG, LATER IN THE WEEK THE WOUND BECAME INFECTED AND REQUIRED TREATMENT.

EE WAS WORKING ON A PLATFORM TO SET UP THE PORTABLE PLANT. HE WAS LIFTING A STEEL PIPE AND LEANING AGAINST GUARD RAIL WHEN HIS RIB ""POPPED"" CARTAGE TORN/ BRUISED.
