EE was performing maintenance on a screen. While switching hard hats, they turned and struck their head on a bracket, causing a laceration. EE sought med. treatment, where they used medical glue to close the wound. Reporting within 10 days.

Employee was performing maintenance on conveyor belt. While removing a bearing with a mallet, a piece of metal broke off, became embedded into the EE's right pec. On 5/13/25 we were notified the employee had a piece of metal removed and was able to continue working. Reporting within 10 days.

Employee was standing on the crusher platform, heating up oil, when they went to step up 3' onto the second platform. EE felt a pop in their left knee. They sought medical attention on 12/06/2024 and was ref. to PT. Reporting within 10 days.
Employee was performing maintenance on a wash plant. While crawling inside, EE swung arm back & struck elbow on an angle iron. On 3/11/24, the incident was reported, the Employee sought medical treatment & was given restrictions to remain off work. Reporting within 10 days - Work Relatedness TBD.
Employee was performing maintenance on a turbine pump. While removing a 25lbs. coupler they felt a tweak in lower back. Employee sought medical treatment and was diagnosed with a strain. On 01/22/2024 we received notice that the employee was referred to PT.

Employee was removing bolts on the counterweight on the excavator. When EE removed the last bolt, the counterweight fell (hydraulic ram bracket failed). The counterweight struck the ladder causing the employee to fall & hit their head. EE was taken by ambulance & was diagnosed with a fractured skull.
Employee removing a pin from a tilt cylinder on the loader and the pin fell causing left hand to be caught between the pin and the loader. The employee sought medical treatment and received stitches.

Employee was trying to pry free a clamping bar on a screen deck using a pry bar. The pry bar slipped causing the employee's right index finger to get caught between the pry bar and the structure of the screen box. The employee sought medical treatment and received stitches.

Employee was removing nut from bolt using cordless impact wrench on bolt & holding nut with box wrench. Bolt, nut & box wrench twisted causing EE's finger to get caught between box wrench & frame of equipment. EE sought treatment & pin was placed in fingertip for fracture.

EE was tightening bolt on screen deck clamp board & felt pain in right shoulder. EE did not seek medical treatment until April 26, 2018. EE was diagnosed with a labral tear on May 4, 2018 & reported back to work with no restrictions. EE will be having surgery in the near future, in the interim EE continues to work normal job.
Employee was lining up a wear plate on a conveyor chute. As ee went to set it down, ee pinched right middle finger between the wear plate & inner lip of the chute. The employee sought treatment & was diagnosed with a fracture as well as a laceration in which ee received stitches.

Employee was cutting a piece of rubber & the utility knife slipped & cut abdomen. Employee sought medical treatment & received stitches.
EE was tightening several bolts that hold on track pads of a drill and strained back. EE did not seek treatment for several months and did not report the injury to mine management until recently. We have received the medical records and EE had a steroid injection. Therefore, we are reporting this claim now as we were unaware that EE sought treatment until recently.
Employee was using a sledge hammer to remove a pin from a hydraulic breaker arm. On a back swing, employee felt pull in right shoulder. EE sought medical treatment and has now been referred to physical therapy.

Employee was welding & received burns to both eyes. He did have a welding shield on. He sought treatment & received eye cream & oral medication.

Employee was torching a tooth off a loader bucket. As he used a small sledge hammer to knock the tooth off the loader bucket, a small piece of metal chipped off of the tooth & struck him in his knee. The small metal chip embedded itself into his right knee.

EE was doing maint. on dragline crane which consisted of lifting and pounding on shaft. Next morning EE had low back pain. EE went to chiropractor.
Employee was trying to make hole larger in piece of rubber-using ulility knife. Knife cut left finger. He received received stitches from MO.

EE noticed pain in right elbow after using various hand tools to repair equipment. EE did not seek any medical treatment until 9/25/09. He was given a wrist splint and diagnoses with epicondylitis. He continues to work his regular job.
Employee was installing battery in generator. Battery fell while employee was moving it around to install it and cut middle finger on left hand. Employee is off work.

While repairing the wiper blade on a conveyor wiper (belt scraper). EE lost grip of 10lb counter weight and the wiper holder pinched EE's left between conveyor belt and angle iron frame of wiper assembly.
Employee was fueling loader & when he was done he was stepping down the ladder & he slipped & strained his lower back.
No special conditions, employee was simply picking up a piece of steel to be used in the plant.

Checking oil on roll crusher engine and as he climbed down ladder from crusher he pulled a muscle in his back causing back pain.
