Injured was operating an angle grinder cutting a belt roller bracket when the grinder slipped and contacted the right thigh. Injury required stitches to close.

Employee stated EE was removing bolts from a cone crusher with a 1 inch impact when EE mistakenly put the impact on a separation bolt. When EE initiated the impact wrench it pinched EE's finger between the frame and the impact wrench.

The employee stated EE was using a hand drill to drill a hole in EE's toolbox when the bit broke. The broken bit struck EE's safety glasses and then EE's eye.

Employee was grinding on a finishing screen, eyes were irritated the next day, went to the doctor and had small pieces of steel in eye.

Technician was installing a rear A-frame for a 745C Cat haul truck, utilizing their service truck hoist and synthetic slings. While maneuvering the A-frame with the hoist, the A-frame shifted, pinching the technicians finger between the haul truck frame, and the A-frame being moved.

The employee stated was installing a roof bolt when EE felt pain in left shoulder.

The Employee was assisting a welder when a piece of slag went behind his safety glasses and into his eye.

The employee was working in the hopper when a welding lead and stinger fell from above, striking him on the head, neck and shoulders.

The employee reported eye irritation the day after working in the shop where welding was taken place.

Employee was using an air arc welder to remove a cone liner when a flash occurred resulting in burns to his face and neck.

Employee was using a torch to cut bolts off the Primary feeder floor in overhead position. Hot slag dripped down into his glove causing a 3rd degree burn to the lateral side of left hand.

Employee was using a power washer to clean the scale, when debris went underneath the safety glasses and into his left eye.

While using an impact wrench on a bolt, a piece of metal spun around and hit first finger on left hand lacerating finger. 12 stitches were needed.

Employee reports was using a hand drill, the bit went through the metal and the drill twisted in his right hand and smashed against his hand. Dx: fractured right ring finger.

EE reported to have burned two fingers while using a cutting torch.

EE reported while using an impact gun to tighten nuts he felt a pain in his left arm below the elbow

EE alleges left arm & shoulder was hurting when he is operating the Mechanical Scaler.

EE alleges a piece of hot slag fell on his foot inside his work boot while welding.

EE was working in primary crusher when he was struck in the right eye by a foregin object.

EE was welding when a large piece of hot slag dripped into his left boot, burning his ankle. EE took off his boot and wrenched his left knee. EE was sitting on bucket, welding, exposing top of his boot.

EE was using a cutting torch when a piece of Slag went into his boot causing a minor burn, EE was sent to the clinic to receive a tetanus shot. On 4/21/06 EE was given a prescription.

Employee doesn't know when or how the accident happened - he thinks it happened on March 3, while bolting. 3/31/06: Physician determined employee's neck problem is job related. Additional x-rays & testing; possible surgery.

EE was moving steel shaft with a crane when the dust cover slid down the shaft and landed on his right foot fracturing 2nd metatarsal.

EMPLOYEE WAS WELDING CRUSHER HAMMERS WHEN HOT SLAG HIT HIS GLOVE AND BURNT HIS HAND.

EMPLOYEE WAS WELDING ON THE CRUSHER WHEN A PIECE OF HOT SLAG DROPPED IN HIS WORK BOOT CAUSING A BURNED RT. ANKLE.

WHILE SCALING WITH THE HYDRAULIC SCALER THE PICK DROPPED OFF AN OVERHANG, BOUNCED OFF THE FACE AND HE WAS BOUNCED AROUND IN THE CAB CAUSING A NECK STRAIN.

ON 5/13/04 EMPLOYEE WAS CUTTING WAREPLATES WITH A CUTTING TORCH WHEN A PIECE OF THE PLATE FELL ON HIS FOOT, RESULTING IN A FRACTURE OF HIS 2ND METATARSAL ON THE LEFT FOOT.
