Fall to the walkway or working surface

The employee was performing maintenance using a wrench. Wrench slipped off a bolt, causing the employee to lose balance and fall backwards into a shelf then onto the ground. The employee tried to brace themselves with their left arm injuring it.

The employee was picking up debris in the shop and got a metal splinter in EE's fingernail, which was removed by medical professional. (Note: The employee was provided with gloves but was not wearing them when the injury occurred.)

The employee was in the shop welding on a secured 988 loader bucket. The employee was working laying down on their left side and started to roll onto their right side and felt pain in their back.

The employee was operating a CAT 988K loader, with left-handed joystick, when the employee turned their hand outward, EE experienced pain. On 6.27.25 employee started physical therapy.

The employee was removing old mops from a 992-loader bucket when a piece of steel chipped off and hit EE's finger.

The employee was replacing hammer on a drill, while removing it from the truck bed it slipped. While attempting to catch the hammer they hyperextended their arm, injuring their right bicep.

Employee was removing a bearing that had been cut with a torch, using a hammer. A piece of steel came off the bearing and struck the employee in the chest. Two stitches used to close the wound.

Employee stated was cutting a piece of belting with a razor knife when the knife broke loose and cut self in the leg. Employee received two stitches to close the wound.

Employee stated EE was operating a skid steer with an extended rake in the bucket, when the rake fell out EE ran over the rake causing the skid steer to bounce.

Employee stated EE was removing bolts from a cone crusher with a 1 inch impact when EE mistakenly put the impact on a separation bolt. When EE initiated the impact wrench it pinched EE's finger between the frame and the impact wrench.

Employee stated was climbing up the steps on a haul truck when employee lost balance and fell.

The employee stated EE was using a hand drill to drill a hole in EE's toolbox when the bit broke. The broken bit struck EE's safety glasses and then EE's eye.

Employee stated EE was climbing into a screen deck to replace a screen when EE cut hand on a sharp rock.

Employee stated was installing a V belt, rolled finger between the sleeve and the v belt.

Employee was using an air impact gun to tighten bolts on Jaw Die when a strong wind blew dust into his eyes. He had a small piece of metal imbedded into the right eye near the iris. Removed FB in ER and antibiotic eye drops given.
