Employee states they were hand scaling and went to swing the hand bar at a rock when the bar slipped out of their hand and caught the right pinky finger between the hand bar and the top rail of the manlift basket. The pinky finger suffered a fracture.
Employee states they "Went to the wrong side of the handrail thinking the ladder was on that side and it wasn't but it was too late". Employee fell off of the ANFO rig suffering a right ankle sprain.
Injured states "I forgot my knife so blaster was throwing their closed knife at pillar next to me which hit and opened on impact then bounce (sic) and stabbed (sic) me in the arm." 3 stitches required.
Injured was cleaning out under some conveyor belts using a water hose when water deflected onto injured's face an into right eye. Injured was wearing safety glasses at the time. Injured went to Dr. on 10/19/21, diagnosed with a corneal abrasion.
Employee was walking on a catwalk when they slipped and grabbed onto the handrail for support. In doing so, a pain was felt in the shoulder. Injured went to doctor on 10/7/21 and was taken off work.
On 8/18/21 employee complained of shoulder pain from driving haul truck. Employee received steroid injection and physical therapy.
Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.
Injured was operating an angle grinder cutting a belt roller bracket when the grinder slipped and contacted the right thigh. Injury required stitches to close.
Injured was using a sledge hammer to loosen wear parts on a VSI rotor. While striking a cast bar, a piece of metal separated from the bar and entered into the injured's abdomen. Injured went to urgent care and received 2 stitches.
Employee reported on 7/14/2021 that EE's back was hurting and had been bothering EE since the week of 6/28/2021, when EE was operating various pieces of equipment. Employee stated EE's back hurt from climbing in and out of equipment. Employee received medical treatment on 7/14/2021 was released to full duty and again on 7/27/2021 when EE was given work restrictions.
Employee stated that while mounting a haul truck, employee bent over to pick up the coffee that employee had placed on the bumper, lost balance, and fell off the truck.
Employee was seated power washing, stood up and felt pain in their right knee.
The employee states EE closed hopper inspection door on sweeper truck when EE felt a pain in EE's elbow. Reportable due to medical treatment on 4.28.2022.
Employee stated had completed setting a pump. When EE turned to step away, EE struck forehead on the fork of the Telehandler.
Employee stated that while operating a loader, the bucket struck a toe on the floor and caused elbow to be stuck between the arm rest and the machine controls, resulting in elbow pain. No medical treatment until 04/12/2021.
Employee stated EE was operating a skid steer with an extended rake in the bucket, when the rake fell out EE ran over the rake causing the skid steer to bounce.
Employee was walking towards EE's vehicle because the mine shut down due to snowy weather conditions. EE slipped and fell on the snow in the parking lot near EE's vehicle. Physical therapy recommended to start on Feb 15.
Employee stated that while changing liners, a rock fell from a ledge, bounced off something, and struck EE in the mouth, breaking a tooth.
Employee stated that EE was dismounting the machine, missed the step and fell.
Technician was removing a transmission plate from under a 980G loader when the plate fell on EE's left arm causing a fracture. The plate was held by one bolt which bent/broke causing the plate to fall.
Employee stated EE was shoveling on the catwalk of the VSI when EE felt a pain in EE's lower back. Employee received medical treatment on November 6.
Employee stated that as EE was climbing onto EE's loader EE's foot slipped off the ladder rung and EE felt a pull in EE's groin.
Employee stated was hand scaling a pillar when a piece of rock fell from above and struck arm.
Employee stated EE was removing bolts from a cone crusher with a 1 inch impact when EE mistakenly put the impact on a separation bolt. When EE initiated the impact wrench it pinched EE's finger between the frame and the impact wrench.
Employee was stepping from the cab to the track of the excavator and rolled an ankle.
Employee was removing an electrical motor using a chain hoist suspended from a cross brace when the cross brace became unsecured and struck the employee.
On 1/20 during the workplace exam, it was discovered the Gazebo Lake located above the mine had emptied through a geological formation its entire contents into the mine. Date and time of the occurrence is unknown, no weekend work. No injuries from this accident.
Employee was bringing 50 pound anfo bags from inside the truck to the tailgate. While bending over, employee twisted and felt pain in lower back. Employee sought treatment with Chiropractor on 12-30-19. No work restrictions given.
Employee stated was climbing up the steps on a haul truck when employee lost balance and fell.
Employee stated that as they were loading a blast hole, a small rock fell from the hole being loaded above and struck them on the neck and shoulder.
The employee stated EE was using a hand drill to drill a hole in EE's toolbox when the bit broke. The broken bit struck EE's safety glasses and then EE's eye.

Employee stated was installing a V belt, rolled finger between the sleeve and the v belt.
Employee stated was dismounting the haul truck, as EE approached the ladder on the front bumper EE fell to the ground EE lost balance and fell.
Employee stated was lifting bags of ANFO and felt a strain in the left shoulder. Went to the doctor on 6/14/2019 and was referred to physical therapy. Was first aid until then.
Employee stated was using a pry bar to clear material from a chute when the bar slipped and employee caught a finger between the chute and the pry bar.
Employee stated ee was scaling in a man lift when the rock ee was prying on forced the pry bar down and caught finger between the pry bar and the top railing on the man lift basket.
Employee was grinding on a finishing screen, eyes were irritated the next day, went to the doctor and had small pieces of steel in eye.
Employee stated EE was driving the man-lift out of the mine when the man-lift hit a hole in the road causing the man-lift to rock and jarred EE's back. No medical treatment until physical therapy was prescribed on March 13.
Employee stated ee was cleaning the concrete pad with a hose when ee stepped off the pad and strained knee.
Employee stated was cutting a rubber spacer with a utility knife when EE slipped and cut hand.
Technician was installing a rear A-frame for a 745C Cat haul truck, utilizing their service truck hoist and synthetic slings. While maneuvering the A-frame with the hoist, the A-frame shifted, pinching the technicians finger between the haul truck frame, and the A-frame being moved.
Employee was pushing a wheel barrel of rock during cleanup and felt pain in neck and shoulder.
Employee stated was standing along a conveyor while it was being started when a rock came off the conveyor and struck them on the hardhat.
Technician was installing a suspension cylinder on a 773 Off Road truck. While attempting to push the rigged cylinder in to place technician felt a pinch and pain in lower back.
Employee stated was using a wrench to loosen bolts on a flange when the employee slipped and cut finger on a bolt head.
Employee stated was using a razor knife to cut piece of belting and cut leg with the knife.
