Showing all 8Miner was using a four inch grinder with a cutting wheel while cutting plate steel. Blade hung up in seam and shattered cutting miners left index finger. Miner received +/- 15 stitches in finger and was released back to duty.
Miner was preparing to cut the inner race of a bearing off of an overflow catch pan on a rip-rap machine, when pressure inside of catch pan forced adjacent bearing off of catch pan housing striking miner in left side causing contusion. Miner was taken to medical facility for observation.
Miner was burning holes under crusher for wear plate installation, when a piece of slag entered his right ear, causing a minor burn. This resulted in a trip to Urgent Care, at which point prescription antibiotics were prescibed.

EE WAS USING 3/8" DRIVE IMPACT TO REMOVE BOLTS FROM GAS TANKS STRAPS FOR REMOVAL OF TANK. BOLT HE WAS LOOSENING WITH IMPACT WAS SLIPPING. HE THEN NEEDED A WRENCH TO HOLD THE BACK SIDE. HE PLACED OTHER HAND ON BACK SIDE OF BOLT TO FEEL WHAT SIZE NUT IT WAS. AS HE PLACED HIS FINGERS ON THE NUT HE ACCIDENTALLY TURNED ON THE AIR GUN SPINNING NUT CUTTING FINGER.
INSTALLING STEP BRACKET ON REAR OF 980 F CAT LOADER. BRACKET HAD THREADED HOLES, PUT BOLT IN BRACKET AND TIGHTENED WITH AIR WRENCH, THE BRACKET ROTATED AROUND AND STRUCK EE ON LEFT THUMB.
