Employee used a 10 foot ladder to climb the bowl mill structure to inspect the top of the bowl mill. As employee was climbing down, accidentally placed left hand near an unguarded drive belt and pulley located approximately 15ft off the ground. Employee received a serious injury to left pinky.

Employee was stacking material with an excavator as ee was lifting the bucket towards the cab a rock flew causing the windshield to break. Employee felt something hit eye.

Employee was vacuuming debris from jack hammer when a piece of debris flew up under his safety glasses. The debris caused minimum irritation. Employee's condition worsened as time went on. Doctors put him on restricted duty to stay away from dusty environments.
Employee was using a cutting torch to cut a return roller bracket off the C-51 conveyor system. He moved his left hand in front of the torch, burning himself. Employee reported to leadman but the leadman did not report to plant manager. Therefore plant manager did not tell safety department until 3/16/15.
Employee was replacing a bearing on the head pulley on C-1. He was chipping on the shaft to slide the bearing on. As he was chipping a piece of metal hit his eye. This caused a large cut on his left eye. He needed two medical procedures to repair the eye.
Employee was drilling holes on Mason sand conveyor to install a belt wiper, while drilling the drill jammed and twisted, fracturing employee's right ring finger.
Employee was replacing cutting edges on a 980H Front End Loader, the employee pushed up on a bolt that slipped out of the cut out, when another employee engaged the impact wrench cutting employee's right palm, employee received six stitches and was returned to work on restricted duty, employee will follow up with doctor on 3-17-2011.
Removing a front-axle of a haul truck and was struck on the right arm when employee cut through the pivot pin. Fractured right forearm.
EMPLOYEE WAS GREASING BULLGEAR AND RIGHT HAND SLIPPED AND RIGHT INDEX FINGER WAS PULLED BETWEEN BULLGEAR AND IDLER GEAR CRUSHING IT. RIGHT FINGER WAS AMPUTATED AT THE KNUCKLE. HE RETURNED TO W ORK ON 2-7-01. HE IS EXPECTED TO RETURN TO WORK FULL DUTY ON 2-19-01.
WAS RUNNING A GRADER NOT BEING USED TO IT JERKING HAD PULLED SOMETHING IN LEFT ARM.
TWO EE'S WERE MOVING A 8" PLASTIC PIPE WHEN ONE HAD TRACKHOE OPERATOR ROLL BUCKET OUT AND BUCKET HIT INJURED'S RIGHT KNEE CAUSING SWELLING. EE WENT TO THE DOCTOR ON 10/3/00. DR SAID NO DAMAGE WAS DO, THAT THE SWELLING WOULD GO DOWN IN A FEW DAYS. EE RETURNED TO WORK.
STRAIN TO RIGHT ELBOW WHILE TRYING TO START AIR COMPRESSOR. INJURY OCCURRED 5/22/00 BUT HE DID NOT WAT TO SEE DOCTOR UNTIL NOW (JUNE 12). DOCTOR DIAGNOSED BURSITIS TO ELBOW AND ORDERED 2 WEEKS REST BEFORE RETURNING TO REGULAR DUTY.
