A CUSTOMER DRIVING A DUMP TRUCK WAS STRUCK BY A TRAIN WHILE ENTERING THE PLANT. HE DID NOT STOP AT THE STOP SIGN. THE TRAIN PUSHED THE TRUCK APPROX. 800 FT DOWN THE TRACKS. THE DRIVER DIED OF RESULTING INJURIES.

Employee lifted a 10 inch pipe into bed of truck and while setting it down the pipe rolled and struck his left wrist causing fracture. employee was released to restricted duty.
Employee stated that after he stepped off of a Cat 980H Front End Loader he felt a sharp pain in his lower back. He was examined at a local clinic and determined that he strained his lower back. Employee was released to light duty and will follow up with doctor on 3-11-13.

Employee was using 56 inch pry bar to move chute on Primary Shaker, the pry bar slipped and employee fell back approximately 3 feet and struck rib cage on middle bar of the hand rail, fracturing ribs 7,8 and9 on his right side, doctor released him to restricted duty.

Employee was helping rotate a suction hose to connect to the a de-watering pump when he said he felt a pop in his right shoulder, he was examined by a physician and was returned to work with restrictions, and will follow up with doctor on May 20th.

EE was replacing a wiper motor assembly on a CAT 385 Excavator. When the wiper was energized, the linkage struck the top of the EE right hand. This resulted in a slight fracture to the third digit and tiny lacerations to the second, third and fourth digits.

The employee was moving an office chair in the Plant Control Tower when she felt a strain in her middle back.
While traveling to a different stock pile, the employee proceed to raise the bucket on a CAT 980H and ""flick"" downward to remove any excess material. The bucket struck the ground and the unit came to an immediate stop,thrusting the employee forward,causing him to strike his chest on the steering wheel. He was wearing his seat belt at the time of the incident.
Pipe casing fell from drill rig onto employees left hand causing lacerations, broken bone and torn tendon in left 5th digit.

EE was greasing standing on a ladder greasing a haul unit in the lube bay. He was standing on the third (3rd) rung from the top of the ladder. When he pulled on the grease line that was attached to the grease fitting, he lost his balance and fell 8 feet to the ground landing between two grease barrells.
Two workers were removing 6' suction lines. The rope slipped & squeezed injured's right hand. He went to the DR on 4/9/2008. The hand was too swollen so he went back to the DR on 4/16/2008 for X-RAYS. He has a fracture on his (R) 4th digit.

Injured EE & another EE were walking between the office and the plant, injured started waving his hand around his face and said something had just stung him on the nose. About an hour later injured's face became severely swollen, injured was taken to the clinic, he was givin a shot and a prescription and was released to return to work.
Employee was changing a tooth on a dragline bucket. He was using a sledge hammer to loosen up the jammed tooth; when he struck the tooth with the hammer a 1/8"" piece of sharp metal chipped off and lodged into his chest causing him to bleed. Employee was then taken to the E.R.
Employee was cleaning up around the plant area with the help of a loader and an operator. The loader operator scooped up a bucket of oversize rock. EE noticed a piece of scrap metal in the bucket.EE pulled the metal from the bucket and a rock fell out stricking him on his right foot.

Employee was walking down the catwalk after checking on pump. As he stepped from the platform approx. 12"" in height he twisted his right ankle causing a severe sprain.
EE was servicing his dragline #190 he went to set on the cat-walk by climbing on the tracks as he pulled his self up. Using the guard rails, the weld broke, causing him to fall 3 feet to the ground.
EMPLOYEE WAS WASHING HER HAUL-UNIT. SHE WENT TO SHUT OFF THE WATER AND DISTURBED A SWARM OF BEES. AS SHE TURNED TO GET AWAY, SHE TRIPPED. AS SHE WAS TRYING TO KEEP FROM FALLING, SHE HURT HER BACK.

EE WAS OPERATING A FRONTEND LOADER. WHEN HE WENT TO CLIMB DOWN HE GRABBED THE HAND RAILS AND MISSED THE FIRST STEP. THIS CAUSED THE OPERATOR TO FALL DOWN THE LADDER MISSING ALL THE STEPS. HE FELL TO THE GROUND 7 1/2 FEET HITTING ON HIS RIGHT SIDE.

EMPLOYEE WAS CUTTING A BOLT WITH A TORCH AND A PIECE OF SLAG CAUGHT THE TIP OF HIS GLOVE ON FIRE. HE RECEIVED A SMALL BURN ON HIS LEFT MIDDLE FINGER. HE WENT TO THE DOCTOR FOR FIRST AID AND WA S RELEASED WITH NO RESTRICTIONS.

MWER WAS OPENING A GREASE BARREL. HIS HAND SLIPPED AND HE WAS CUT ON A SHARP SURFACE. HE REQUIRED STITCHES TO CLOSE THE WOUND.
EE WAS OPERATING A FRONT END LOADER. HE COMPLAINED OF NECK PAIN FROM THE ROUGH RIDE. HE WENT TO A CHIROPRACTOR FOR TREATMENT AND RETURNED TO WORK WITH NO LOST TIME OR RESTRICTED DUTY.
