Over-exertion (Not Elsewhere Classified)

Employee was on the secondary shaker removing the middle screen of the top deck. The employee was repositioning himself to allow the screen to be pulled higher on the deck, as he stepped and turned his right foot got caught on the upper screen causing his right knee to twist. Employee was released to restricted duty and get rechecked by doctor on 7/25/2008.

Fall to the walkway or working surface

EE was going to fill fuel tank on fuel truck, when he slipped and fell on a broken step on the walkway at the fuel Island. EE fell on right shoulder, causing pain. He will be receiving physical therapy for 2 weeks and follow-up with DR. on 6/2/06. EE did not report injury until 5/11/06. Had Dr. appt. on 5/15/06.

Employee was checking on a broken travel chain on dragline when he slipped and fell in the engine compartment, hurting his right knee (contusion). He went to doctor and was given light duty until Jan. 22, 2005 at which time he may resume regular duty.

Exiting front end loader from ladder to get off and slipped & fell to the ground, causing headache. Was taken to hospital and released to full duty as of 6-11-04.

EE WAS CHECKING ENGINE OIL ON 4600 MANITOWOC DRAGLINE AND SLIPPED AND FELL HURTING LEFT THUMB.

EMPLOYEE WAS GREASING BULLGEAR AND RIGHT HAND SLIPPED AND RIGHT INDEX FINGER WAS PULLED BETWEEN BULLGEAR AND IDLER GEAR CRUSHING IT. RIGHT FINGER WAS AMPUTATED AT THE KNUCKLE. HE RETURNED TO W ORK ON 2-7-01. HE IS EXPECTED TO RETURN TO WORK FULL DUTY ON 2-19-01.

EE WAS ALIGNING TRACK PAD WITH COMEALONG AND FELT A SHARP PAIN IN HIS LOWER BACK. EE WENT TO THE DR. ON 9-20-00 AND THE DR. SAID HE HAD PULLED A MUSCLE IN HIS LOWER BACK. EE WAS GIVEN MEDICATI ON AND TOLD TO COME BACK ON 9-22-00.
