Showing all 5EE was working on our rock chopper. He was removing a rock from the rollers. When the operator rolled the rollers ee glove got caught in the rollers and pulled his hand down into the rollers.
Employee had gone to see a doctor over the weekend claiming an accident where his left index finger was caught between rocks while working on the chopper. His supervisor had not been informed of an accident. Doctor visit showed fracture to left index finger.
Employee was driving a loaded pan around a turn and the pan flipped over and his foot got caught under the rollover protection and crushed his foot.
EE WAS TAKING D-7 TO SHOP TO FUEL & SERVICE. HE HAD TO TRAVEL DOWN A STEEP INCLINE AT BACK OF RAW MATERIAL SHED. EE HAD THE DOZER IN A HIGH GEAR AS HE WENT DOWN STEEP INCLINE, THE BLADE WENT I NTO GROUND STOPPING THE EQUIPMENT. EE'S HEAD HIT THE WINDSHIELD BUSTING THE GLASS & INJURED EE.
