Employee was an air impact with reducer and socket. They went to the place the tool in place to tighten a bolt and felt a pop in their elbow. They were given self care treatment, but on 3/16/2026 they sought medical treatment and were given medication for treatment.

Employee was attempting to blow off a turntable in the packhouse between the grate using an air lance. As the employee was pulling the air lance from the grate they felt a sharp pain in the right shoulder. The employee received medical attention, and been place on restricted duties.

An employee was trying to clear a lump of hardened material from above the silo rotary feeder. They used an air lance through a port to get the material to start flowing. The silo became pressurized with the use of the air lance and cement came out of the port and blew into the employee's eye. The employee received a chemical burn to their eye.

Employee was using an impact wrench equipment with a swivel and socket to tighten one of the bolts that rend the knives. The accident occurred when the socket separated from the swivel and flew into employee's face causing a laceration on the upper lip.

An employee was using an air hammer to clean bolts at the secondary sizer. They were sitting on a 10 inch lip of the sizer bending over. The employee reported back pain and sought medical treatment the next day. They were placed on restricted work duties.

The employee was cleaning the Pre-heater tower from stage 5. A long lance was used in order to create distance and allow the employee to stand aside while cleaning. At some point, while unplugging the port, material exited the port getting into the employee's eyes.

Contractor with Metal Surgery (E230) was grinding a weld on a girth gear, when a metal shaving flew up underneath the employee's face shield and safety glasses and got in their eye. The employee sustained a scratch to their cornea.

EE was using a compressed air hose removing a blockage in a lime dust system when the hose became crimped and then releasing air outwards, blowing dust from off the work landing into the EE's right eye.

Tech using 2 different size jack stands to support the bucket of a WA-900 and did not relieve the pressure from the hydraulic line. When they broke the control valve loose it caused the bucket shift and fall off of the jack stands. When the bucket fell the dog bone linkage swung down and hit the tech just below the knees. Tech sustained injury to both legs.

The employee was cleaning the riser on the 4th floor West side, with a WAMA gun, when hot dust flew out of the pot hole and get into their left eye. The employee was using all PPE at that time. Immediately, they flushed their eye. They experienced redness in their eye on 08/02/2017.

Employee was welding when a piece of slag burned through pant leg and fell down into boot causing a burn to right foot. Employee was treated with first aid on site until 1/4/16 when they requested medical treatment.

The employee was removing a snowman, when a built-up fell and hit the lance of the water pressure gun. This caused that EE's left hand turns. EE felt a little pain, but EE thought it was nothing. On October 8, 2016, the employee mentioned to EE's supervisor what happened, but told supervisor that it was nothing. On October 24, 2016, EE's supervisor sent EE to a doctor.

Employee was installing a vibrator on a coal car while using an impact gun. The exhaust air pressure blew debris into right eye. Employee thought it was dust and it would come out on its own. Ended up being 2 small pieces lodged in eye that was removed by the eye doctor.

Electrician was working in a shallow trench tightening 2" conduit, kneeling next to a ground wire. Backhoe operator, 25 feet away, snagged the ground wire with bucket, lifting the ground wire out of the ground and pinning the knee of the electrician against the ground wire and conduit. Employee burned hand with an open torch flame while cutting/grating metal pieces inside the finish mill. He explains that while cutting metal pieces the hose got caught on something and as he pulled to free it, the flame went under glove and burned the top part of his hand. Restrictions as follows: keep wound clean and dry with tolerated use of (L)hand.

Installing a chute on top of a feeder in the coal mill. The sling got hung up and the foreman told his crew to get a look from the back of the chute. The injured miner did not move(he does not understand english well). There was a problem with the rigging & the chute slid and hit injured below his knees breaking both legs.

Erecting scaffold in cyclone when a clinker fell from an overhead section of the cyclone. The clinker landed on the employee's top right side of the hard hat. It also struck his right shoulder, right hip and left thumb causing a fractured shoulder and wrist.

The reason for accident is the head ache ball on the crane got hung up on edge of roof transferring some of the weight to the roof. structure. when the load came uncaught the load lurche down and bounce due to boom deflection. Ee was sitting on a beam and was caught between the beam and the load.
