Employee was using a 1" air impact wrench to remove bolts from a cutting edge lip protector on a 993K CAT loader. EE was removing a bolt and it began to rotate. EE pushed up on the bolt and bumped the rotation of the air impact hoping the bolt would lock back into position but instead the bolt spun causing a laceration to EE's left index finger. This resulted in 17 stitches.

Employee was installing an anchor bolt in the #5 heading. He was pushing the bolt to the top, stopped up pressure to turn the anchor, then continued pushing the bolt up. He caught his left middle finger between the wrench and the anchor resulting in a laceration requiring 3 stitches.

Bates Contractor was operating Fletcher (DDR) walk thru roof bolter installing wire mesh in the #4 entry on Northeast Mains. His left arm became entangled in the rotating drill steel/chuck causing amputation of the arm at the elbow.

Bates Contractor stated he drilled starter hole and laid his starter steel across & behind the drill head. Installed his finishing steel, proceeded to drill the hole and noticed the starter steel was lying on the drill head. When he grabbed the steel it caught the lip of the canopy jack tubing mashing his fore finger against the scissor bar resulting in a laceration and 3 stitches.

Bates Contractor was installing roof bolts with truss bolter and had the canopy raised and was drilling a hole. The collar on the canopy shell had raised up with canopy. He had his hand on the joy stick with his finger extended out. When the collar fell it caught his left index finger between collar and bottom of shell resulting in a fracture and stitches.

Employee was in the process of placing his finisher and pusher in the drill chuck when the steel clamped together he released the steel and began rotation. The cuff of the glove caught in between the drill chuck and pusher pulling the glove tight on his hand resulting in a fracture.

Undetected material slid down the T-bar and slid down the mass boom of the T-bar striking the light frame causing the rock to break into small piece striking him on the left shoulder. When he moved away he struck his head on the canopy driving his hard hat down on his left ear causing a laceration resulting in stitches.

While installing a roof bolt, a piece of drill steel fell off the tray. The employee went to catch the steel with his left hand while still pushing up the bolt and caught his left thumb between the boom, the tray and drill steel resulting in stitches and a fracture to his left thumb.

Employee was working on a water clamp and had his right hand resting on the lifting arm next to the canopy post. The other operator came over to assist and started to lift the drill head higher. This mashed his fingers and resulted in fractures of his middle, ring, and pinky fingers and stitches.

Employee was in the process of drilling anchor holes for tailpiece with a jack hammer when he pulled it up he felt pain in his lower back running down through his legs. He continued to work until 1/5/2012, when the doctor removed from work.

On 11/21/11 employee was hanging a block curtain in #2 entry when the drill opr. turned the cable reel on. His feet were in a loop of cable and swept him off his feet. He stated that he felt pain in his left shoulder. He had surgery on his shoulder on 4/3/12. We obtained confirmation on 4/3/12 that this was related to the injury that occurred on 11/21/11.

Employee states while pulling drill steel out of the rib bolt hole the steel fell out of the wrench and became wedged in the jack area. Employee went to remove the steel by moving drill boom and mistaking pushed the up pressure, pinching his left middle finger.

Employee was helping load reducer with a hoist when reducer fouled on rear of mantrip. When he tried to free the reducer, the hoisting strap slipped causing it to move suddenly and catch his right ring finger against the reducer and mantrip. Resulted in a fracture and required stitches.

The miner was de-energized. EE was moving cables to prepare to cut a corner.He put the rope on the bolt on top of the J-hook and went outby the miner.He started the miner and the rope slipped off and he leaned in to pick the rope up and was pinned against the rib with the boom of the miner.He freed himself and walked to the front of the miner.

The injured employee was checking the dump valve for the dust box on the left side roof drill. When the stab jack was inadvertently raised, the flapper on the dust box closed on his left thumb resulting in a laceration and fracture to his thumb.
