EE was inspecting a noise on a shaft under sand classifier. EE placed right hand on the guard as they leaned in to take a closer look and the key stock on the end of the shaft caught the tip of a glove and pulled a finger into the guard. EE received a 1/4" laceration to the side of the right pointer finger.

EE water blasting on tower and has alleged injury to right shoulder while positioning water blasting lance. Additional report of continued pain on 6/17/20. Taken for med. eval. and referred to specialist. Became MSHA reportable on 7/10/2020 with the Specialist positive diagnosis of rotator cuff tear on 07/10/20 and prescribed physical therapy.

EE1 put bolt-inner bearing housing. EE2 loosened outer fastener;bearing house pulled out.EE1 said stop;EE2 re-tightened fastener. Bolt in-place for alignment of shaft/flange. Vibration/movement from impact wrench may have let shaft spin on bearing. Pendulum effect occurred;paddles contacted EE1. EE exited tub w/o help; walked 300' to truck. EE1 stated legs hurt;driven to hospital.

Employee was engaged in operating a portable hydraulic jack. When they and their partner found that they did not have the right size jack, they placed a large nut between the unit and the equipment housing to act as a spacer. When they powered the unit, the nut shot out striking the employee in the right index finger

EE was driving the 834B rail car mover to the main parking area. At the point the employee crossed under a conveyor, a piece of limestone buildup fell from the conveyor frame above and landed against the front windshield. The windshield broke in one piece and folded in half striking left hand. EE suffered a laceration to the left 5th digit and left forearm.

On 3-18-16 employee was knocking in a thermal coupling and sprained EE's right wrist. It turned into an MSHA reportable on 3-30-16 when it was found EE had a fracture.

EE FELT SOMETHING IN EE'S EYE AND RUBBED IT WITH EE'S HAND EE FLUSHED EE'S EYES FOR 15-20 MINUTES, WE DIDN'T FIND ANYTHING IN THERE BUT I TOOK EE TO HOSPITAL FOR WASH OUT. E.R SAID EE SCRATCHED EE'S EYE AND SENT EE BACK TO WORK. I BOUGHT EE SOME GOOD EYEDROPS EE IS FINE AND BACK TO WORK

Employee was using a hand drill to tighten wire holding a scaffold plank in place. The tip of his glove got caught in the wire and pulled finger into drill bit. Drill bit cut the 2nd finger of his right hand.

Employee using cutting torch. There was slag on the end of the torch and employee knocked slag off with hand. His glove had a hole on the left hand index finger and when he hit torch with left hand acetylene flared and burned his left hand and index finger.

On 4/19/11 at approximately 11:57am, EE was operating the crane to relocate the pump in the west pit. As he swung to the left, the crane turned on its left side. EE was taken to the hospital.

EE was replacing a wiper motor assembly on a CAT 385 Excavator. When the wiper was energized, the linkage struck the top of the EE right hand. This resulted in a slight fracture to the third digit and tiny lacerations to the second, third and fourth digits.

At approximately 3:30 PM on 8/6/10 EE & co-worker were assigned the task of cleaning of SC#1 belly pan. EE was working at removing buildup of raw shale from the upper section of the belly pan with a pneumatic chipping gun when a large piece of raw shale broke loose and slid down the sloped belly pan landing on the top of his right foot.

Employee had started the engine on the water pump and went to engage the clutch. When clutch was engaged the coupling broke. As the coupling came apart, it distorted the shape of guard, pinching his thumb between the clutch and guard.

At approximately 9:00 PM on 4/14/2010 while working at the emergency hopper, employee was using a 12 ft. long, 1/2" diameter air lance to help wet raw shale from sticking to the hopper walls. Employee was stretched out pulling on the lance when he felt a sharp pain in his lower back.

Miner was a bystander where welding on a piece of 30" pipe was being performed. Miner was exposed to welding flashes and injured eyes. Miner claims to have been turning as welding took place.

Employee was using a cutting torch without wearing gloves, when the oxygen lead came off the torch, causing a 2nd degree flash burn to the employees left hand.

Injury was grinding on a piece of angle iron near the bucket wheel. He felt a piece of slag hit him in the chest and then it bounced off entering the face shield then stricking him in the right eye. He stated to me that he was wearing his safety glasses and safety shield. He immediately reported this to the plant Mgr.

Employee & co-worker were changing cutting edges on a loader. One of the bolts had slipped out of slot. Employee reached up to push it back in & his co-worker turned it with an impact wrench causing the bolt to spin in his hand, causing a cut on right index finger & right thumb. He was wearing heavy gloves, but it went thru it.

While checking on tower plug, cannon blaster went off spraying hot materical on employee. He had protective equipment on but hot material came in contact with skin. also when he took protective hood off, he hit a pipe and had a laceration on his forehead. Employee had 1st and 2nd degree burns and a cut on forehead that required stitches. Employee is on light duty.

Employee was mixing lime with water for a heat test. He went to dump the sample and the mixture blew out of the container knocking his safety glasses off and getting into both eyes causing chemical conjunctivitis.

EE was air-arc gouging on frame damage to a scraper. The area was very small and at or above head level causing his head to be surrounded by billowing air, fumes dust and grit. EE felt that he had gotten metal in his eyelashes and then in to his left eye. The first real discomfort he felt was on Saturday, 5/2/09.

Was using magnetic drill press, used his hand which was gloved, to sweep away shavings. While the drill was running, his glove got caught in the drill, pulling his finger into the drill. He suffered an open fracture to his left index finger. He required stiches.

The employee was aligning two bolt holes on a screw shaft. He stuck his left middle finger in the bolt holes and asked another employee to turn the shaft. This caused a laceration of the left middle finger. The injury required sutures.

Employee was using a grinder with a cutting disc to cut a piece of angle iron. As the Employee was finishing the cut, the disc caught part of the angle iron causing the disc to put across his left arm. An inspection of the grinder showed the ginder was guarded and all safety equipment ppe were being used.

Employee was removing the wings of a sand dewatering screw. He was using a cutting torch to remove the bolts that held the back plate in place. During this process the torch hose got caught on something. While trying to free the hose, he was positioned under the plate. The final bolts broke, causing the plate to land on top of him.

Employee was using impact wrench to remove door on engine compartment on Front-end Loader. The rotation of the wrench was opposite as expected and pulled finger in, and hammered against a piece of metal causing a laceration requiring 5 stitches.

EMPLOYEE WAS CLEANING AROUND THE SECONDARY CRUSHER. A ROCK CAME OUT OF OR OFF OF THE CRUSHER AND STRUCK THE EMPLOYEE IN THE RIGHT EYE. THE EMPLOYEE WAS WEARING THE PROPER PPE. THE EMPLOYEE SUFFERED FROM HYPERLLA IN RIGHT EYE. SAFETY GLASSES PREVENTED SERIOUS INJURY.

Employee was lancing material out of the bottom of #8 GCT when a chunk of material fell hitting the lance. The lance came up & struck his jaw on the left side causing pain & swelling. He was transported to hospital and treated for bruising & pain. He was released & returned to work on light duty.
