Crew was spot bolting 5R on #2 section. Both operators had finished putting up bolts, and operator went to inch tram controls. Offside bolt man walked back to get bolts for his side. The operator started the bolter and began tramming forward, that is when the bolter slid off a small ledge, on top of employee's foot.

Employee was installing belt hangers when his drill steel became stuck. The employee tried to spin the steel out, while placing his hand on the drill steel, his hand got caught, wrapping his arm around the steel causing a compound fracture to his right forearm.
Employee was preparing to bolt top when his hand got caught between pusher steel and wrench causing a laceration. The employee received 8 stitches.
EE walked into a red zone and was struck by the boom of the miner and rib.
EE was bolting on operator side when finisher became lodged. EE took pusher by hand trying to dislodge the finisher from the top & was unsuccessful. He then held the pusher with his left hand and operated the drill pot with his right hand running the controls. When the drill pot was raised to the top, he tried to hold it over with his left hand & mashed his finger receiving a cut.
Employee was bolting top and twisted knee.
Employee working on air pump. He reached into the pump to check for mud. He accidentally hit air valve, turning pump on. Has 3 broken fingers, cuts, & lacerations. Unknown hand.
Tailgate on truck would not open. The driver of the truck asked grader operator to push against the tailgate with the front part of the grader. When this happened the EE was holding a rock between the grader and the tailgate his left fingers got trapped between the grader and the rock. E.R. was visted by the EE.
Employee started to back the continuous miner out of cut when the outside bit struck his foot.
EE stated that while he was standing beside of the roof bolting machine the machine rolled back and ran over his left foot.
Employee stated he was helping move continuous miner to #6 entry, said he was walking beside of a miner when it kicked sideways pushing him against the rib injuring his left knee.
The employee was preparing to spray the roof and ribs to fireproof for a charging station. He checked the machine for blockage and plaster sprayed into his eyes.
Employee put the drill steel in pod, hit the rotation and a burr on the drill steel caught his glove and twisted his arm around the drill steel.
Employee was watching the cable while the roof bolter was being trammed when the cable hanger broke causing the cable to sweep employee off his feet. He fell onto his side and landed on his rescuer.
