Showing all 40At 8:30 am, a Heavy Equipment Operator who has worked at Sandy Ridge for 8 years with 10 months experience in the mining department was starting a 4-horsepower gasoline powered water pump when EE strained EE's lower back while pulling on the engine starting pull cord.
Employee was hoisting a 55 gallon barrel with a crane and a "choker" sling. During the hoist, the barrel slipped out of the sling and smashed employee's finger.
Got finger stuck in bag clamp and it popped the finger nail out. EE had 2 stitches.
Dozer drove over a rock and jarred employees back. EE did not report injury and sought chiropractic care the next morning. Injury was reported to management on Saturday the Sept 2nd. We are unsure if chiropractic care counts as a recordable so we are reporting this just to be safe.
EE was operating a dozer while backing up and drove up onto a large chunk of slate. The dozer slid off the chunk and jostled the employee in the cab of the dozer. The employee felt pain between shoulder blades directly after.
Employee was doing some live work and placed employee's hand in a hangar bearing and the finger got pinched. Employee received 10 stitches.
Driving scraper and drove over the top of a big rock. The scraper rocked from side to side and the employee was jostled around the cab of the vehicle. The employee felt a sharp pain in lower back. The employee was wearing a seat belt.
EE was cutting a piece of conduit with a band saw. While doing so the saw jumped and cut his finger. He was received 4 stitches on his right index finger thus constituting in an MSHA reportable incident.
Backing over already ripped material using a D-8 Dozer. Backed into a large chunk of material. Received a compression fracture in the back from the jarring.
EE was operating blade and hit a washed out area in the road. This jarred his neck and a doctor put him on restricted duty of not operating mobile equipment. This affects normal job operations thus constituting MSHA reportability.
EE was running a dozer and was shifting from forward to reverse. He was pulling against the shifter and his shoulder started to hurt while shifting. Put on restrictions affecting operators daily job task, this constituting an MSHA reportable.
EE was cutting a coupler in half with a band saw, he turned the saw on and the blade caught and kicked up. When it kicked up, it came down and cut the EE's finger.

Around 11:30 am the subject was at the back of the maintenance shop telling another employee that he felt dizzy and had a sharp pain in his left arm and shoulder. Ambulance was called and he was taken to the hospital.
Gave OK to start up piece of equipment, then placed arm in to get chunks out and feeder grabbed his glove and cut off small part of index finger.
EE using jackhammer to bar at hammer mill. Turned to move hammer to left side of mill door and got his left pinky finger knuckle pinched between bottom left of the door edge and jackhammer.
EE was using 1/2"drill, drilling out fluid bed dryer grid. Drill jammed, causing the drill and EE's R wrist to continue in a clockwise motion. Twisting caused wrist to rapidly go into strained position, causing wrist strain. Wrist was immobilized with a splint, thus constituting MSHA reportability.
EE hit chin on bulk bag machine support arm when bag strap fell off, causing arm to move upward. Received stiched to cut.
Mega tex was running. Shut if off and boot of mega tex slipped into pipe, mega tex was winding down, reached down to grab sck in pipe and mega tex pinned left arm.

EE entered guarded area of palletizer, with machine running, attempting to straighten up a product box, when machine struck EE under left arm and pushed him into support post. Injuring (broke) 3 ribs on R side, bruising lung and injuring pneumothorax.

Operators trying to unplug overs pipe that leads to turbo mill. When screen moved, it caught EE's right pinky finger between screen outlet and discharge pipe. Finger tip fracture and stitches required.
dusty and noisy 1/17/07: called Rick to obtain more information. EE was running a hoist, shive 056got bound, because of noise & dust, EE could not hear radio and the cable struck his thumb.
Employee got caught in the guard rail at hooder. He was pinned between the hooder arm and the guard rail with his head facing toward the palled that was in the hooder. The stainless steel can was directly below him and the chain still wet when other person came.
EE was pumping water from pit, left area for an hour, returned and pump was still running but no water coming out the discharge hose. Shut pump off and uncoupled the discharge hose. Hot water from pump sprayed on EE's forearms adn stomach/chest resulting in 1st and 2nd degree burns on 10% of body. THERMAL BURN
EE drilling hole thru metal angle iron using electric drill when drill bit caught in metal and pulled drill out of hands, fracturing right hand.

Got hand and forearm caught between roll and rewind roll when rubbing finger tips on rewind roll. Roll took hand and forearm in roll.
EE WAS OPERATING D9 DOZER, BACKING OVER BIG CHUNKS OF CLAY, JARRING BACK. EE SOUGHT CHIROPRACTIC CARE THUS CONSTITUTING MSHA REPORTABILITY.
EE WAS DRILLING A HOLE INTO AN ALUMINUM CABLE. A SMALL ALUMINUM FRAGMENT BECAME LODGED IN EYE. EMBEDDED - SURGICALLY REMOVED, AND PRESCRIPTION WAS PRESCRIBED. THUS CONSTITUTING MSHA REPORTABIL ITY.
DRILLING HOLES WITH DRILL PRESS AND WIPED FILINGS OFF BIT WITHOUT TURNING POWER OFF. BIT CAUGHT GLOVE AND PULLED FINGER INTO BIT. RIGHT INDEX FINGER WAS LACERATED FROM TIP TO SECOND JOINT.
EMPLOYEE WAS GRINDING ON METAL AND DID NOT NOTICE UNTIL THAT EVENING THAT HE HAD METAL IN LEFT EYE.
EE WAS USING DRILLING PROBE EXTENSION, FAILED TO PUT CHUCK IN. SLAMMED HAND INTO A RAIL OF THE KKELLY BAR, BREAKING HIS RIGHT THUMB.
WHILE EMPLOYEE WAS CHECKING TO SEE IF CHEMICALS WERE REACHING CLAY, HE REACHED INTO MOVING AUGER & CAUGHT MIDDLE FINGER OF LEFT HAND, PARTIALLY SEVERING FINGER TIP.
HE WAS RUNNING 627 SCRAPER, SOMETIME IN THE P.M. HIS LOWER BACK STARTED HURTING.
EE WAS BLOWING DOWN HIS WORK AREA AND THE AIRHOSE CAME OFF THE BLOWPIPE, LOST CONTROL OF IT, SNAKING AROUND. IT DIRECTED A SHORT BLAST OF COMPRESSED AIR INTO HIS EAR.
EMPLOYEE TURNING THE RIPPER SHANK OVER TO WELD ON IT. WHEN HE RAISED IT WITH THE CRANE, IT BECAME UNBALANCED AND SLIPPED AND HIT HIS FOOT, FRACTURING HIS TOE.
