Showing all 7Welding in S-3 discharge chute. Lowered piece of 16" x 2' patch down to the tacked into place. Was then welding in chute between EE's legs and sparks caught pant leg on fire.
An employee working for a contractor was using a 6" inch DeWalt angle grinder on the corner of a new cold fee bin to prepare to weld. When he began to grind the surface in a vertical fashion, the grinder "kicked" back at the employee striking him above his right eye and on the right side of his nose.
EMPLOYEE RECEIVED FLASH BURNS TO HIS EYES. HE WAS HELPING A FABRICATOR WITH SOME REPAIRS ON A CHUTE, WHEN THE SECONDARY FLASHES OCCURED FROM THE WELDING PROCESS. THE INJURY WAS NOT EVIDENT UNTIL THE FOLLOWING DAY. HE THE RECEIVED EYE DROPS & CREAM AT THE HOSPITAL. HE RETURNED BACK TO WORK WITH NO RESTRICTIONS 3 DAYS LATER. 3/26/04
RUST INSIDE HELMET OR BLOWN UP INTO HELMET.
TWO EMPLOYEES WERE REPLACING THE WING PULLEY ON THE BERM STACKER. BOTH ENDS WERE SUPPORTED WITH A CHAIN HOIST. EE RAISED HIS END AND THE HOIST CAME UNHOOKED SMASHING HIS HAND BETWEEN THE PULLE Y SHAFT AND THE CONVEYOR FRAME.
EMPLOYEE WAS USING AN OVERHEAD HOIST TO MOVE A STEEL PLATE. HE HOOKED HOIST DIRECTLY TO PLATE, THE HOOK SLIPPED AND THE PLATE STRUCK HIS ELBOW CUTTING IT.
