Employee was cutting a piece of conveyor belt with a pocket style utility knife. The knife slipped out of the belt and struck the employee in EE's left knee lacerating it. The employee received 4 sutures to close the wound.

Employee was on the 8 wash screen changing poly decks. Employee was using a screwdriver to pry pin out of the poly screen. Screwdriver broke and the employee fell forward into the broken screwdriver puncturing their left wrist.

Employee was preparing to removing a camlock from the rear of the water truck. It was stuck, so they attempted to tap the fitting with a hammer and it had pressure built up and it struck the employee in the mouth.

THE DRIVER WAS ATTEMPTING TO CLOSE THE SLIDING WINDOW ON THE WATER TRUCK FROM THE SEAT & AFTER BEING JAMMED ON THE 1st ATTEMPT, PULLED HARDER ON THE 2nd ATTEMPT & THE WINDOW BECAME FREE & THE DRIVER LOST HIS BALANCE & FELL OUT OF THE CAB ONTO THE GROUND LANDING ON HIS FEET & THEN FELL FORWARD ONTO A PALLET OF SCREEN CLOTH CUTTING HIS LEFT KNEE.

THEY WERE CHANGING TEETH OF THE BUCKET OF THE 750 KOMATSU EXCAVATOR. EE WAS HOLDING THE BAR ON THE KEEPER, CO-WORKER WAS USING AN 8# HAMMER TO HIT BAR. HAMMER SLID OFF BAR STRIKING EE'S RIGHT HAND.

TWO EMPLOYEES WERE REPLACING THE WING PULLEY ON THE BERM STACKER. BOTH ENDS WERE SUPPORTED WITH A CHAIN HOIST. EE RAISED HIS END AND THE HOIST CAME UNHOOKED SMASHING HIS HAND BETWEEN THE PULLE Y SHAFT AND THE CONVEYOR FRAME.
