I had changed the cables on a crane from a single to a double. The crane operator boomed up as my hand was on one of the cables causing might pinky on my right hand to be pinched between the pulley and cable.

Showing all 10Employee had been welding and helping another welder weld on a drainage pipe throughout the shift. Employee was diagnosed with flash burn.
Employee was knocking the pin out of the bucket from the stick of the backhoe. The stick moved causing the pry bar to pinch between the stick and bucket. The pry bar then hit employee in his right collar bone.
While using a grinder, employee got some metal in eye.
WIRING IN FEED HOPPER & TRANSFER BELT WITH THE GENERATOR RUNNING
CUTTING WELD OFF DRAGLINE GEAR. GEAR WAS OUT OF MACHINE BUT THE GEAR ITSELF WAS NOT SECURED & WHEN THE WELD WAS KNOCKED OFF THE GEAR SLIPPED OFF THE SHAFT HITTING HIM IN THE HEAD & PINNING HIM UNDER THE GEAR.
WHILE OPERATING A DOZER, HIS BACK STARTED HURTING. HE SAID THAT HE THOUGHT IT WAS DUE TO A PREVIOUS INJURY ON 3-28-02. AT THAT TIME HE WAS WORKING FOR LEHMAN-ROBERTS & WAS COVERED BY OSHA. HE TOLD THE DR. IT HAD BEEN HURTING FOR ABOUT TWO WEEKS& A NEW CLAIM WAS STARTED. REPORTED TO US ON 2-24-03.
PUSH DOZER BUMPED SCRAPER& SCRAPER OPERATOR RECEIVED MUSCLE STRAIN IN BACK.
EE IS A CONTRACTOR THAT WAS CUTTING UP & HAULINGOFF THE SCRAP IRON IN THE BONEYARD. HE WAS CUTTING THE COUNTERWEIGHT OFF AN OLD TRUCK CRANE. HE HAD CUT ONE SIDE LOOSE & WHEN HE CUT THE OTHER SIDE HE COUNTERWEIGHT FELL & HE DID NOT HAVE HISFOOT OUT OF THE WAY. ABOUT 10K PDS OF COUNTERWEIGHT.
