Struck against a moving object

MSHA inspector issued citation forcing us to file even though we don't believe this is a recordable. Employee stated they had soreness in shoulder on Thursday, May 1. There was no lifting, pushing, pulling or striking incident that caused this shoulder soreness. States ""I drove over a bump in my path and it jarred me"".

Struck against a moving object

While checking rotation of the piggy backs in the pit, the pig was rotating backwards. Employees switched the electrical wires inside the junction box. When turning the power back on it caused an electrical short which made the employees run into one another. Our employees fell to the ground injuring their right shoulder. The employees were located 200 feet away.

Injured employee assisted in maintenance of log washer. Employee was standing on walkway holding journal of log washer. As EE was doing this another employee was in the tub of the log washer removing the bolt. As the bolt was removed the journal became unstable, pinching three fingers of the employees left hand.

Employee states EE and another employee were carrying a pipe. Each employee was positioned on an end of the pipe. one employee suddenly dropped EE's end resulting in EE's right shoulder being strained.

Employee was training employees on how to train a conveyor. He states he was using a hammer to tap the rollers for adjustment. He completed this and walked to his truck and as he began to open his truck door he felt back pain.

The employee was using a utility knife to cut a piece of conveyor. The knife slipped and lacerated the employee's right leg.

Employee states he was pulling on a suction hose of a water pump that was stuck in the mud and injured his lower back.

Employee states he was installing flashing on a conveyor belt to prevent spillage. As he was pulling on the flashing to properly place it his right hand went across a piece of jagged metal resulting in a laceration to his index finger on his right hand.

Employee states he was operating a water wagon to wet the roadways. In the evening the employees back began to bother him.

EMPLOYEE WAS AT THE REAR SECTION OF AN 8"" PIPE THAT WAS BEING PULLED FROM A TRENCH BY A 824 JOHN DEERE LOADER. THE REAR OF THE PIPE SUDDENLY CAME UP OUT OF THE TRENCH AND STRUCK HIM IN THE CHEST.

Employee states he was securing crane to lowboy trailer. As he was throwing chain through the hook of the crane to secure felt a pain in his right shoulder.

Employee was striking a U joint with a hammer on a Terex Scraper when a piece of metal came off of the U joint entering his right index finger.

Struck by falling object

Employee states he had positioned a 6X10 sheet of metal in a vertical position in a hopper to weld. He had placed (3) 2x2's in a horizontal position with one end against the sheet of metal and the other against the wall. As he was climbing out of the hopper on a ladder sheet of metal came unsecured and pinned him against the ladder.

Employee states he was walking down stairs of control tower when he stepped on a wrench and twisted his ankle.

Employee was replacing flooring at the primary plant. The employee was using a pry bar to move a steel plate when he felt pain in his back.

Employee states he was removing hydraulic pump and it slipped and caught his finger between metal lines and cut left middle finger.

CUTTING WELD OFF DRAGLINE GEAR. GEAR WAS OUT OF MACHINE BUT THE GEAR ITSELF WAS NOT SECURED & WHEN THE WELD WAS KNOCKED OFF THE GEAR SLIPPED OFF THE SHAFT HITTING HIM IN THE HEAD & PINNING HIM UNDER THE GEAR.
