Employee was changing a roller on a field conveyor. Employee was holding one side when it suddenly fell pinching the middle finger of right hand between the roller and conveyor frame. This resulted in a contusion to right hand and also injured the fingernail of the middle finger of the right hand .

EE was pulling off a Roller from the conveyor to replace, as EE grabbed it to remove as EE and helper pulled it EE's finger was caught in roller, it crushed and cut middle right finger

I had changed the cables on a crane from a single to a double. The crane operator boomed up as my hand was on one of the cables causing might pinky on my right hand to be pinched between the pulley and cable.

This was not a Memphis Stone employee and there was no incident. A crusher operator discovered a customer(truck driver) in the cab of truck unresponsive and notified the foreman in which emergency medical help was summoned through the use of 911. Paramedics arrived and transported to the hospital where passed away. There was no event that occurred which caused this.
Employee was putting a battery on a John Deere excavator when the battery suddenly blew up. The employee received initial evaluation on the day of occurence for ringing in ears and given no diagnosis. Employee was referred to an audiologist for evaluation.

Employee states he was removing a cutting edge from a bucket. He was using an impact wrench to remove a bolt that secured the cutting edge on the bucket. As he was removing this, the nut began to spin, and he grabbed it to secure it from moving. When he did this it caused a laceration of the index finger of the right hand.

Employee was knocking the pin out of the bucket from the stick of the backhoe. The stick moved causing the pry bar to pinch between the stick and bucket. The pry bar then hit employee in his right collar bone.

Employee states that he had a back injury on October 9, 2006 while moving crossties at a conveyor belt. He did not report this when it happened. It was reported to us after the first of the year.
EMPLOYEE WAS UNHOOKING CHAINS FROM A CONVEYOR COVER THAT WAS BEING LOADED ON TO A FLATBED TRUCK. EMPLOYEE HAD UNHOOKED CHAINS AND WAS GOING TO UNHOOK OTHER CHAINS WHEN A BOOM POLE ATTACHED TO A FRONT END LOADEDER STRUCK HIM IN THE LEFT SHOUDER.

WHILE USING A PRYBAR TO ALIGN A ENGINE GOING INTO A BACKHOE, THE PRYBAR SLIPPED AND CAUGHT HIS LEFT LITTLE FINGER BETWEEN THE PRYBAR AND THE ENGINE.CONTUSION AND LACERATION TO THE LEFT LITTLE FINGEER AND BROKE THE END OF FINGER.
