Employee was working under the 7 ft crusher using a porta power jack, this became dislodged and struck EE in the lip causing 6 stitches to be administered at the hospital. It was found that the foot of the jack failed.

Showing all 12Employee was replacing a cutting edge on a loader bucket. He was removing a bolt using an impact wrench. He placed his right hand on the underside of the bucket against the bolt head. He accidentally hit the trigger of impact wrench causing the bolt to spin in his hand cutting through his glove causing a laceration to his middle finger resulting in four stitches.
Cutting bolts overhead, slag from cut dropped and blew into ear. Slight, burn to ear.
The point (tip) of the rock breaker hammer was jammed in-between two of the fingers at the primary feeder. The EE placed his hand on one of these fingers. The rock breaker hammer moved down and struck the back of the EE's hand. We brought the EE to get X-rays on the hand which came back negative - nothing broken. Hand was bruised.
Employee was grinding a weld that was left when we removed the simplicity screen. The grinder slipped out of his hand and cut his left knee.
While EE was grinding steel, a piece came up and under his full face shield and glasses and lodged in his right eye.
LOADER OPERATOR WENT TO GARAGE WITH LOOSE CUTTING EDGE, MECHANIC WAS IN THE BACK OF THE GARAGE. OPERATOR PICKED UP AN IMPACT WRENCH TO TIGHTEN BOLTS & PUT HIS HAND UNDER EDGE TO HOLD BOLT WITH SHARP EDGE. BOLT SPUN & CUT HIS FINGER WITH NO GLOVES ON.
WELDING OVER HEAD, PULLED SHEILD UP HITTING WELDING SLAG, WENT OVER PRESCRIPTION GLASSES. HOT SLAG IN LEFT EYE.
Miner was flame cutting steel on plant from a catwalk when slag dropped onto leg and into cuff of boot, burning ankle area.
Employee was laying on his back and side welding the frame and a piece of slag got under the helmet and into left eye burning eye.
WHILE GRINDING OFF OLD CLIPS, GRINDER SLIPPED & CUT FINGER.
KNEELING ON KNEES ALL DAY WELDING. AT THE END OF SHIFT FELT PAIN IN LEFT KNEE WENT TO THE DOCTOR. DIAGNOSIS IS A RUPTURED LEFT BURSA.
