EE was servicing a pump. While doing so, the EE slipped catching their leg on a piece of angle iron resulting in a laceration needing sutures.

Cutting bolts overhead, slag from cut dropped and blew into ear. Slight, burn to ear.

Fall to the walkway or working surface

EE arrived at the work site prior to the start of his shift. EE proceeded to head towards the mechanic shop EE parked his car in a location that had been plowed last week it was not the designated parking area. The EE got out of his car and proceeded to walk over an area of accumulated snow and ice. While doing so lost his footing and fell on his side.

Injury reported to company via MSHA inspector and Vermont DOL 8 days after ee stopped showing up for work for reasons unrelated to any injury. No report of injury from personnel onsite during any interview. We do not know anything other than ee states ""banged knee"". He has sought medical attention without our knowledge, have not seen medical reports.

Employye was using a utility knife to cut a piece of grease line off a bulk roll, the blade of the knife came out of the holder a cut the employees right thumb(employee is left handed) . The employee recieved three stitches in his right thumb and returned to duty.

Struck by falling object

The crusher hopper was cleared of material in order to perform repairs to the crusher. The employee was performing repairs when the wind blew a chip curtain causing a loose rock behind it to fall into the crusher striking the employee in the back of the head.

Caught in, under or between a moving and a stationary object

Several employees were attempting to remove a 4' section of I beam frozen into ice. They used a bobcat to loosen the beam and when it broke free it caught the employee's finger between the beam and the wall.

Contact with hot objects or substances

Employee was laying on his back and side welding the frame and a piece of slag got under the helmet and into left eye burning eye.

Struck by falling object

EMPLOYEE WAS SETTING A STEEL PLATE ON A HOPPER. ONE END OF THE PLATE SLIPPED OFF THE EDGE OF THE HOPPER CAUSING THE OTHER END TO FALL PINCHING THE RIGHT PINKY FINGER, RESULTING IN A LACERATION REQUIRING STITCHES.

EE ALLEGES THAT HE HIT A POTHOLE ON THE ROAD ANDSUFFERED A BACK INJURY.

EE WITH THE HELP OH ANOTHER EE WERE LIFTING A 150 LB TOGGLE SEAT UNDER THE JAW CRUSHER. THE TOGGLE PLATE SLID TOO FAST AND PINCHED THE FINGER OF THE EE. TOGGLE FELL ABOUT TWO FEET.

EE WAS USING WELDER, PUTTING IN A NEW MOTOR BASE ON CORRODED METAL WHEN A PIECE OF RUST BOUNCED INT0 HIS EYE.

Accident type, without injuries

EE WAS CUTTING A BOLT OFF OF A CONVEYOR CHUTE. THE BOLT TRAVELED TO THE ADJOINING CHUTE IGNITING IT AND CAUSED FIRE TO TRAVEL UP THE CHUTE INTO THE POLYDECK SCREEN.
