Showing all 42Removing broken master link bolts - D10T track. Elevated portion on the service truck crane and a crib block. When lowering track to ground, engaged boom in order to elevate the track off the crib block. Boom had a delayed due to a now diagnosed missing sensor spring. The boom raised unexpectedly, caused tension on the crane cable and the cable whiplashed. Laceration to forehead.
Employee was backing up EE's run in the shot material on the Caterpillar D10T Dozer when a rock rolled out of the wind roll and the dozer backed up onto the rock and then slide off the rock jarring the employee.

EE WAS PUSHING FILL WITH A D11 DOZER, HE CALLED FOR THE FOREMAN TO COME GET HIM. EE STATED THAT HE COULD NOT STAND THE BUMPING OF THE DOZER. HE STATED THAT THE PAIN WAS FROM A PREVIOUS INJURY FROM A PREVIOUS EMPLOYER. EE WOULDN'T GO TO THE DOCTOR, BUT HE DID GO HOME.
Employee was cutting a tree down with a chain saw, when a limb flew back and hit him above the left eye. Four stitches to close laceration.
