Caught in, under or between a moving and a stationary object

Employee was changing lift cylinder on D-9 Dozer, as EE was putting cylinder in there was slack in cable and the cylinder moved back in place causing the cylinder to smash finger. Employee had surgery on left pinkie finger, it was smashed & cut.

Employee was dressing around face-up of mine area when the corner of his dozer blade caught a gas line causing the dozer to catch on fire and he jumped out and broke left leg and brunt both hands.

Employee was dumping a 777 haul truck, when the fire extinguisher became dislodged and fell onto the frame of the truck. When the employee lowered the bed of the truck, the fire extinguisher was crushed, filling the cab of the truck with the contents of the fire extinguisher.
