After cutting a speed reducer loose from 5 head drive the speed reducer slid from head drive falling on employee's left arm. Due to his injury employee received a fracture to his left arm, employee has returned to his regular work duties.

Employee was in #5 entry installing roof bolts, during this time employee raised head on bolter, not aware of where his right hand was placed employee caught his right 3rd digit finger with the canopy of bolter causing injury to finger, employee has returned to work on light duty due to injury on 12/5/11.

Employee was drilling top with drill steel from roof bolter when drill steel kicked out striking employee in chin, employee has received 7 sutures in chin. Employee has returned to work with no restrictions.

A bolt and plate was laying on the ground. When employee let down head on roof bolter it caught the roof bolt plate causing the roof bolt to fly up striking head just above right eye. Sutures required, employee has returned to regular duty with no restrictions.

Employee was checking to see what was wrong with head on roof bolter. EE asked helper to raise bolter head so that he could look, unaware of having placed left hand at pinching point, resulting in mashing ring finger tip off and cutting middle finger below nail bed, later having to remove middle finger below nail bed.

While operating roof bolter, employee was drilling hole into roof when drill steel broke, cutting left index finger. Employee received 11 sutures and been placed on light duty.

Employee was grinding on lo lo structure when something went into his left eye. Employee went to doctor on 5/19/10, object was removed from eye. Employee returned to work with no restrictions, employee was wearing safety glasses, a guard was on the grinder at time of accident.

Employee cut left leg shin while using grinder working on lo lo structure, requiring 4 sutures. Employee has returned to reg. duty with no restrictions.

Employee was helping on roof bolter when a channel strap fell hitting left index finger. Sutures required, employee has returned to regular duty.

Employee was bolting top in when drill steel fell hitting right middle finger fracturing it. Employee returned to reg. duty with no restrictions, just received x-ray report 6-11-09.

While operating roof bolter, wrench used on drill steel broke, hitting employee left hand. Employee received a fracture to his 4th metarsal, employee became a lost time on 10-09-08.

Employee was pulling stuck roof bolter with miner when chain broke hitting employee on left side of forehead. 8 sutures required, employee has returned to regular duty.

Roof bolt operator drill steel got hung as he reached to get wrench drill, steel fell out hitting top of left hand, employee has fractured his 4th metacarpal shaft. Employee has return to regular duty.

Employee was bolting top when something got into his right eye, employee was wearing safety glasses at the time, employee has been treated and returned to regular duty with no restrictions.

employee was bolting top in #4 heading when a piece of drawrock fell off canopy striking left knee, until has been place on light duty.

Employee was bolting top with a single head roof bolter in low coal, employee let the head jack down on his left knee, employee was placed on light duty with a bruised left knee.

Employee was operating a roof bolter when his glove got caught between drill steel and roof bolt twisting his right thumb and fracturing it, employee has returned to his regular duty.

Employee was bolting top when drill steel got stuck in top. Employee turned to get wrench when drill steel fell out and hit top of right hand on the 2nd and 3rd knuckle. He received a fracture and has been placed on light duty.

Employee caught his left hand in the drill steel while installing roof bolts using gloves. He injured three fingers on his left hand. The glove was caught on a burr on the roof bolt itself and twisted his fingers.

EMPLOYEE ALLEGES HE TWISTED HIS LEFT KNEE WHILE INSTALLING ROOF BOLTS

Employee was installing roof bolts when he turned around and struck his left eyebrow against the ATRS arm (T-bar).

CAUGHT RIGHT ARM BETWEEN ROOF AND TOP OF BOLTER AND STRAINED AND BRUISED ARM

Employee was struck by a piece of drawrock while installing roof bolts in the #3 entry.

Employee alleges he strained his neck while adding an extension on a roof bolt machine.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAWROCK, ON THE RT. FOOT, WHILE OPERATING THE CONTINUOUS MINER . NOTE: FIRST DAY OFF WORK WAS 10/18/04.

EMPLOYEE SWUNG THE HEAD OF THE BOLT MACHINE TOWARDS HIMSELF AND STRUCK HIS RIGHT KNEE.

EMPLOYEE WAS STRUCK ON THE LEFT ARM BY A PIECE OF BELT RAIL WHILE PILLARING OPERATIONS WERE BEING CONDUCTED. THE CONTINUOUS MINER DRUMS THREW A PIECE OF BELT RAIL THAT WAS COVERED WITH MUCK.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAW ROCK ON THE LEFT HAND WHILE OPERATING THE CONTINUOUS MINER. THE ROCK WAS APPROXIMATELY 2" X 3" X 1/2" THICK.

Employee caught his right gloved hand between the drill steel and the roof bolt to be installed.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAWROCK WHILE INSTALLING ROOF BOLTS. (THE ROCK FELL AS THE DRILL STEEL WAS PLACED AGAINST THE ROOF TO DRILL THE HOLE).

EMPLOYEE WAS STRUCK BY A RIB ROLL WHILE OPERATING A CONTINUOUS MINER. NOTE: EE WORKED UNTIL 8/2/04 THEN STARTED MISSING TIME.

EMPLOYEE WAS STRUCK BY AN UNDETECTED ROOF ABNORMALITY.

EE WAS STRUCK BY A PIECE OF DRAWROCK WHEN HE SET THE A.T.R.S.

EMPLOYEE CAUGHT HIS RIGHT SMALL FINGER BETWEEN THE LEVELING ARMS ON THE BOLT MACHINE.

EMPLOYEE WAS STRUCK ON THE RIGHT RING FINGER BY A PIECE OF ROCK THAT FELL FROM THE ROOF.

EMPLOYEE WAS STRUCK BY A SMALL PIECE OF DRAWROCK APPROXIMATELY 3" X 6" X 1/2" ON THE RIGHT CHEEK CAUSING A CUT THAT REQUIRED SUTURES.

EMPLOYEE ALLEGES HE WAS USING A HAND TOOL (GRINDER) AND STRAINED HIS BACK. EE WORKED UNTIL 12-1-03 AND STARTED MISSING TIME.

EE ALLEGES HE BURNED HIS EYES WHILE WELDING ON THE CONTINUOUS MINER.

EE ALLEGES HE STRAINED HIS LOWER BACK WHEN HE CHANGED DIRECTIONS ON THE DOZER AND IT JERKED HIM BACK.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAWROCK WHILE ROOFBOLTING. THE ROCK WAS APP. 6" X 4" X 1" AND HIT THE EMPLOYEE ON THE LEFT CHEEK BELOW THE EYE.

EE WAS STRUCK ON THE LOWER BACK BY A RIB ROLL ON001 SECTION.

EE ALLGES HIS RIGHT KNEE WAS STRUCK BY THE HEAD OF THE ROOF BOLT MACHINE BEING OPERATED.

EE WAS STRUCK BY A PIECE OF DRAW ROCK APP. 6' LONG 3' WIDE RANGING FROM 2" TO 4" THICK. EE WAS PERFORMING MINER HELPER DUTIES WHEN STRUCK BY THE ROCK.

EMPLOYEE WAS STRUCK ON RIGHT HIP AND LEG BY RIB ROLL.

EMPLOYEE WAS STRUCK BY A RIB ROLL ON BOTH LEGS.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAWROCK WHILE ROOF BOLTING.

OBSERVING ROOF BOLTER-STANDING TOO CLOSE TO MACHINE MACHINE BACKING OUT AND CAUGHT HIS ARM BETWEEN RIB AND THE ATRS.

EE RECEIVED FLASH BURNS FROM WELDING OPERATIONS WHILE REPAIRING #002 FACE DRIVE.

EMPLOYEE WAS STRUCK ON THE RIGHT ANKLE BY A DRAW ROCK.

EMPLOYEE WAS MOVING ROOF BOLT MACHINE AND CAUGHTHIS LEFT FOOT BETWEEN THE DECK AND MINE FLOOR.
