While using a slate bar to pry rock loose in the conveyor; the bar slipped, striking him on the right ear.

Accident type, without injuries

A roof fall was discovered in the #1 panel off the Big Bear sub-mains during a weekly examination. The fall was apx. 90' long X 20' wide X 4' to unknown height. The fall was dangered off and reported to MSHA. The roof fall will not be cleaned up and a ventilation supplement has been submitted and approved.

Caught in, under or between a moving and a stationary object

Employee was leaning into mantrip giving out ear plugs unaware of his right foot was half way under mantrip, mantrip moved catching right foot under mantrip and twisting foot. Employee received a fracture to his right ankle. Employee has returned to work with a full release.

Employee was loading tray on roof bolter with bolts when he caught his left digit finger on tray while loading. Employee received hairline fracture to his finger, employee has returned to reg. duty with no restrictions.

Struck against a moving object

On 8/14/2013 employee was traveling on rubber tire buggy when he ran over a rock buggy bounce causing head to hit roof top, employee continue to work received a full release 8/27/2013.

Over-exertion in lifting objects

On 4/23/13 employee was picking up a rock at the lo lo when he twisted and strained his left arm employee continue to work until 08/01/2013 having a herniated disk he became a lost time accident due to this injury.

On 5/18/12 employee stated that he was informed by his personal physician that he has pneumoconiosis.

Caught in, under or between a moving and a stationary object

The employee got off a fork and got on another forklift and backed into the forks of the other machine, the fork struck his left leg. He went to the doctor and had 12 sutures placed in the cut he received. He returned to work the next scheduled shift. Full duty.

Struck by falling object

Employee was checking to see what was wrong with head on roof bolter. EE asked helper to raise bolter head so that he could look, unaware of having placed left hand at pinching point, resulting in mashing ring finger tip off and cutting middle finger below nail bed, later having to remove middle finger below nail bed.
