Miner was sitting on apron of screen before the Roto-Lift chipping clay out of fins while screen deck was not operating, but still energized. Roto-Lift started turning while miners feet were between fins, causing left leg to become crushed between Roto-Lift fin and frame of screen. EMS was called and miner transported to the ER.
While loading 14-foot jumbo steels onto the rack, the jumbo began reversing; a steel caught on rock, sprang free, and struck the IP's left knee.
while changing teeth and cutting edges on 988H loader 23.3804 eq #. a wrong bolt was inserted in one of the holes by mistake and while removing it EE cut EE's left ring finger. first aid was rendered and later in the morning went to Urgent Care where they cleaned the wound and did 4 stitches. no restrictions and returned to work same day
Miner was taking the pin retainer cap off of an excavator bucket (heavy round collar weighing about 30-40lbs). EE had to remove three bolts holding the collar on. It released easier than expected. EE's fingers were crushed between the collar and the bucket, breaking EE's left ring, and middle, fingers.
EE was using Metabo Grinder in an inverted position and cutting against the direction of the blades rotation. the blade kick backed, causing laceration to the left index finger.
Miner was attaching lifting apparatus to pipe on the ground to excavator when pipe shifted and caught miner's finger tip.
A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).
EE was performing grinding operations, the following morning EE felt irritation in EE's eye. It was discovered that EE a small piece of metal debris in EE's eye.
Employee was standing next to a welder torching a bolt. Welder turned hit employee's left hand and burned left hand. Burn caused a second-degree burn. No lost time.
EE was welding under the feeder at the primary plant at Penn MD Quarry when a welding bead dropped and fell down into EE's boot, burning the top of EE's left foot
Injured was operating the tail side shearer while they were cutting out on the head. A rock came off of the tail drum and struck EE on the right thumb causing a laceration resulting in 9 sutures.
Employee was bolting when a piece of the rib slid off the ventilation tube and struck EE's neck and left shoulder. Employee was diagnosed with neck contusions and a possible concussion. EE was taken off of work the same day 5/19/26.
While using a one-handed reciprocating saw, the blade hopped out of the rubber liner that was being cut and punctured a maintenance employee's hand.
The injured was drilling into the rib to install a rib bolt, when EE was bringing the drill head back toward EE, EE caught EE's right ring finger between the inner mast and inner carriage area of drill head and the drill station frame.
pushing material with a rubber tire dozer was backing up and ran over a rock causing machine to bounce and cause back strain.
Employee was jack hammering during clean up and felt a piece of material or dust fall in EE's eye. Immediately washed-out eye but swelled up overnight and following morning took EE to the clinic to be seen and employee had a scratch to EE's eye. "Abrasion of left cornea"
EE was operating the headgate side of the shearer, while snaking back into the coal block employee was standing 5 shields away when EE was struck by a piece of coal measuring 3"x4"x1" thick resulting in a fracture of the Maxillary sinus.
Employee stated EE was in the process of bolting the first cut out of 3L on the 007 MMU. After EE finished the first row, draw rock feel from the roof in by the ATRS. Resulting in a piece of draw rock falling into EE's drilling station and striking EE on the right leg.
While performing geotechnical soil borings for new project driller got EE's tip of the left pointer finger smashed. Finger was caught in-between the vice and drill rig while attempting to remove pin to open the vice up to pull augers. Finger needed first aid which was three stiches and driller returned to work after receiving first aid.
Had EE's finger accidentally struck with a chipping hammer that another employee was using. Causing a cut 7 stitches, and bruising.
At 13:00, Haul Truck 474 was being loaded by Shovel 71. During third loading pass, a large rock was loaded into the bed of the haul truck. Upon impact, the haul truck operator reported feeling pain in their right shoulder. The operator advised that the shoulder pain increased in severity. The operator then contacted the facilitator to report the issue.
A miner performing water blasting activities in the preheater tower riser area when handling the water lance and when the water supply valve was opened got a kickback force causing the gun section of the lance to strike on EE's right ear, knocking EE to the floor. EE was transported to the hospital, where EE received eleven stiches in the ear and was treated for a concussion.
Employee lost control of grinder and cut EE's leg.
Right thumb was pinched between square hub bolt head and access removal slot wall in the cage mill housing, resulting in a laceration that required stiches.
EE was pulling shields in, one advanced striking employees feet resulting in 2 broken toes and 2 broken metatarsals.
Employee stated, EE was drilling a bolt hole to strap a kettle bottom. A piece of the kettle bottom broke off striking EE on the right index finger, causing a fracture and seven sutures.
EE was assisting in pulling rocks off the miner when EE moved between the scoop bucket and rib. The miner operator then trammed the miner backwards, hitting the scoop bucket which then caught employee between the scoop and rib.
Employee was center bolting when EE was reaching for plate and EE's finger got caught in drill guide. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
A Mill Diagnostic mechanic was guiding a coupling guard into position using crane assistance. The mechanic attempted to lift the bottom edge of the guard to maneuver it over the adjustment bolt. The guard was set back down, the mechanic's left hand was still positioned underneath, resulting in the left index finger being caught and pinched between the adjustment bolt and the guard.
An EE was in the process of cutting a vertical wall cut using a Sky stone wire saw. The saw had been running at normal startup amps. During this time the employee walked behind the saw at the exact time the diamond wire failed and struck the employee on EE's right leg. Causing a laceration to the mid thigh. After physician consultation amputation near the knee was completed.
Employee stated that as EE was installing a rib bolt EE felt a sharp pain in EE's middle / left lower back.
EE was assisting the plant welder, with fabricating brackets for the bin gate clam shell. While using a drill press and grinder small metal particles collected on the sleeves of the EE shirt. EE wiped EE's face with EE's sleeve contaminating EE's eye with the small metal particles. EE did not seek medical treatment until 5/12/2026.
Mill utility employee was jackhammering in Silo 4 when the jackhammer became stuck. While attempting to free it, the jackhammer suddenly released and dropped, pinching the employee's right pinky between the jackhammer and a grizzly bar. The employee received medical treatment- reinserted fingernail and stitched it to hold in place. Was told to keep dry and clean.
While performing overhead hydro-blasting operations in the hopper area of the scrubber, hardened material adhered to the scrubber wall became dislodged and fell from the scrubber wall. The falling material struck the employee on the left hip area.
Employee was drilling concrete and cleaning holes to install studs. Concrete dust got in face and eyes.
Welding on jaw of crusher, after shift realized scratching in eye from foreign object due to welding.
EMPLOYEE STATES EE WAS PUTTING EE'S SECOND STEEL AND A ROCK FELL HITTING EE'S HAND CAUSING A LACERATION TO EE'S RIGHT MIDDLE FINGER
A rock rolling around on the crusher return belt came off and struck the employee in the foot. Causing a break to the 2nd metatarsal of the right foot.
Employee was running shearer when a rock came off the drum and struck EE's hand. Mining systems was not a factor, compliance with rules and regulations was not a factor. PPE was a factor.
I.E. was standing on the longwall pan line when 1 of the recovery bolters turn on EE's cable reel causing the bolter cable to hit Employee in EE's lower legs causing EE to fall hitting EE's left arm on the pan line. Employee had 8 stitches in EE's left arm.
Injured employee was attempting to install a roof bolt when the bolt bent, separated from the drill chuck, and struck EE's right forearm resulting in a fracture and laceration.
Employee was getting sloped plant area fixing washouts and ready for hydroseeding. Employee backed up to far to an area that had a weathered berm and slid off backwards when the wet ground shifted causing the dozer to fall off an approx 7' drop. Dozer landed on the back cab and remained in vertical position. Employee suffered compressed vertebrae fracture
Technician pinched EE's hand while using an air impact gun causing a laceration that required stitches.
Jetter hose had a weak area that was not recognized in initial shift inspection. Upon use, the hose burst injuring operator on the left hand. Was treated at local medical center for hand injuries and referred to secondary care at burn center in Augusta. Care was advised to apply ointments and wrap. Return for follow-up in two weeks.
Employee grinding iron while wearing welding hood and safety glasses. Spark struck ground, rebounded under hood and glasses, and entered left eye. Evaluated by physician: shallow metallic foreign body in left cornea. Topical anesthetic applied; foreign body removed without tissue removal. No pain or residual issues reported.
EE climbed into the chute of the packer system to lube the roller without locking out/tagging out packer. EE's left hand was caught between the conveyor and the closest roller.
Employee reached into bag machine to remove jam when the belt grabbed EE's glove and pulled it into the belt pulley. The injury occurred when the employee reacted by pulling EE's hand out.
Essentially it came down to the operator of the portable band saw becoming complacent. The trigger was accidentally depressed with right hand while the left hand had came in contact with the blade of the band saw. It was the end of the shift and worker was cleaning up shop. The worker received a total of 6 stitches to the left index finger.
EE was cutting a steel manifold from the side of 10SC001. While getting to the bottom of cutting the brackets off, the manifold broke loose unexpectedly and dropped 3 feet to the catwalk. The EE's foot got tangled in the manifold's valves and dragged EE down the catwalk. This resulted in a twisting of the EE's left ankle.
glove got stuck between 2 drill steels and EE accidentally hit the rotation when trying to get it out resulting in 5 stitches and a fracture all on EE's left little finger.
