Worker was climbing a step ladder when it shifted out from underneath them causing them to grab the catwalk with their left hand which caused a severe muscle strain to their left shoulder.

Employee was working in hopper to C-24, removing frozen material. While lifting a piece with a co-worker, the co-worker's side slipped out of co-worker's hand catching EE between the material and a piece of steel.

Our employee, was washing off crusher deck when a bolt fell out of the crusher box striking them on the left shoulder and their neck.

The employee was using a Skill Saw to cut plastic sheeting when the saw bounced back and sliced employee's thigh.

Caught finger between hook and roof area of cable while changing drill hammer.

Worker got cut on finger required three stitches

Chisel tip went into cheek of employee.

While kneeling on cat walk employee used right hand for support and without realizing finger was through it, stood up lacerating right index finger

Employee was moving wooden blocks used for cribbing and weighing approximate 30 lbs., when employee felt a pop in elbow. The blocks were being moved on the back of employee's service truck to access a container.

Tripped in pot hole in parking lot at 4:00 AM. Didn't report it till 8:30 AM.

While air arcing plate off dozer blade plate had stress on it and sprung out at top hitting employee in head. Causing laceration to scalp.

Employee backed onto 3/4 rs pile to dump, pile was undercut and caved, flipped truck over once landing on roof.

Employee was working at the recycle plant, when he slipped off the crusher and fell onto the ladder. He hit a kick plate and got a contusion on the right side, between his rib & hip.

Employee was emptying garbage & stacking pallets when he twisted his lower right back.

Employee was doing winter maintenance. He cut a piece of steel and was going to install it, when steel slipped and fell on his right foot, causing contusion.

Employee was adjusting dirt shoe spring; he was down on his hand and knees. He turned the wrench once and he felt his back go out.

Employee went to assist in replacing rear cover plate on dozer; plate on floor jack on edge. While jacking up, he lost control of plate and his left thumb was pinched between plate and a solid object.

Employee states rust flew up under goggles into his right eye while using hammer. Visions blurry on right side of eye.

EMPLOYEE WAS OPERATING LOADER THE SEAT SPRUNG HIM UPWARD. AS THE SEAT BELT CAUGHT HIM AND YANKED HIM BACK IN THE SEAT AND SOMETHING POPPED IN HIS BACK.

EMPLOYEE SLIPPED OFF FUEL TANK GETTING INTO ASPHALT PLANT TRUCK. HE GRABBED THE HANDLE BAR BEFORE LANDING ON THE GROUND & HE PULLED A MUSCLE.

EE WAS STRIKING HOPPER WITH A PRY BAR AND THE BAR GOT STUCK IN FROZEN MATERIAL, EE LOST HIS BALANCE AND PRY BAR CAME BACK AND HIT HIM IN THE FACE.

EE WAS CLIMBING BETWEEN WALL & SCREEN STRUCTURE. HIS HARD HAT SHIFTED AS HE WAS CLIMBING AND HE HIT HIS HEAD ON THE COUNTER WEIGHT GUARD ON THE SCREEN. PUT A SERIOUS CUT IN HIS HEAD.

EE STEPPED ONTO LOOSE GRAVEL AND HE SLIPPED INJURING HIS BACK. EE WAS WALKING FROM PARKING LOT GOING TO CHANGE CLOTHERS FOR WORK.

WHILE EMPLOYEE WAS WETTING DOWN SHOT THE SPRAY MECHANISM CAME OUT OF ITS HOLDER WHILE EMPLOYEE WAS USING IT. THE FORCE OF THE WATER PRESSURE CAUSED EMPLOYEE TO HIT HIS ARM ON THE PLATFORM RAIL ING.

EMPLOYEE WAS DOING MAINTENANCE UNDER CONVEYER #1AND HE SLIPPED ON MUD AND SLID INTO WASHOUT GULLY JAMMING HIS LEFT KNEE.

EE STEPPED OFF THE BACK OF THE WELDING TRUCK ONTO UNEVEN GROUND AND SPRAINED HIS RIGHT ANKLE.

EMPLOYEE WAS WORKING INSIDE SCREEN CHUTE AND A STEEL PLATE WITH RUBBER PAD FELL ON HIS HAND AS HE WAS EXITING THE CHUTE.

EE WAS WALKING TO SHOP, HE SLIPPED ON ICE AND HIT HIS RIGHT WRIST ON ANOTHER EE'S THERMOS BOTTLE. WAITED UNTI 17TH, HAND AND FINGER SWOLLEN-WENT FOR XRAYS.

EMPLOYEE WAS TRIMMING THE SKIRT ON THE CONVEYER AND THE UITILITY KNIFE SLIPPED AND HE CUT HIS FINGER. IT REQUIRED 11 STITCHES.

EE WAS TRYING TO REMOVE BOLT ON THE PRIMARY CRUSHER ALONG WITH INJ EE INJ EE'S THUMB WAS STRUCK BY THE SLEDGE HAMMER WHILE REMOVING THE BOLT

EE CUT RIGHT HAND WHILE REACHING INTO A TOOL BUCKET ON A RAZOR KNIFE BLADE WHICH WAS NOT FULLY RETRACTED REQUIRING STITCHES.
