Fall from ladders

Worker was climbing a step ladder when it shifted out from underneath them causing them to grab the catwalk with their left hand which caused a severe muscle strain to their left shoulder.

Employee was working in hopper to C-24, removing frozen material. While lifting a piece with a co-worker, the co-worker's side slipped out of co-worker's hand catching EE between the material and a piece of steel.

Employee was moving wooden blocks used for cribbing and weighing approximate 30 lbs., when employee felt a pop in elbow. The blocks were being moved on the back of employee's service truck to access a container.

Employee backed onto 3/4 rs pile to dump, pile was undercut and caved, flipped truck over once landing on roof.

Fall from machine

Employee was working at the recycle plant, when he slipped off the crusher and fell onto the ladder. He hit a kick plate and got a contusion on the right side, between his rib & hip.

Employee was doing winter maintenance. He cut a piece of steel and was going to install it, when steel slipped and fell on his right foot, causing contusion.
