A supervisor was working on writing an employee review in the office. As another individual walked past the office, they heard a loud noise and went into the office to see what it was. They found the supervisor lying on the floor actively having a seizure. Sometime during the event the left ear and left head suffered lacerations that required stitches.

Employee was kneeling down to inspect the latch on the shovel dipper. As employee was getting up employee felt pain in right leg. The employee has a fractured right fibula.

The employee was cleaning pond over flow drain. The pipe inlet has an expanded metal barrier to catch large debris from entering the inlet. The employee was reaching down to remove large debris from barrier and was bit on EE's right index finger by a venomous snake hiding under the debris. The worker was wearing rubber style gloves when bit.

While closing set of rear compartment doors on Fuel/Lube Truck, gust of wind blew door abruptly causing left pinky to be smashed into other closed door. Individual refused to go see a doctor that day, but on Monday April 9th, requested to go get looked at by doctor. X-Rays indicated dislocated pinky tip and doctor put back in place.

Employee was sitting in supervisor's pickup and had some type of medical episode. Employee was unresponsive, CPR was initiated immediately. Employee was pronounced deceased by Aircare paramedics.

Employee was working and began to feel weak and tired. A coworker recognized the situation and call the supervisor who is an EMR. They determined that outside assistance was needed and called 911. The local EMS arrived and determined the employee was possibly having a heart attack. The employee was treated at the local hospital for a heart attack.

Employee was unloading cam frame bolts (approx. 45 lbs. each) from a plastic box supplied by the mine. He stepped into the box to reach the bolts and noticed a bite/sting on his leg. He keep working. Later the bite site became painful, swollen and abscessed. He reported this to his boss 9/3/14 and was instructed to go the occupational medial clinic 9/4/14.

At approximately 2:00 am, a Kress coal hauler was observed off the road in an MSHA pond. Upon investigating the truck was found to be stuck in the mud and the operator was unresponsive. The operator was later pronounced deceased by the Justice of the Peace.

While driving a rock truck the employee noticed that his speech was slurred and that his face felt different than normal. Employee was transported by company vehicle to the hospital. He was diagnosed with a light stroke.

Employee backed into position to have his 777G rock truck loaded. When the PC2000 operator completed the loading process he attempted to notify the employee that the truck was loaded. After no response it was noticed that the driver was slumped over. Rescue efforts began. Employee was pronounced dead at the scene by paramedics.

Employee was milling on rack pad (Windy Conditions, Milling, & Sandblasting). Felt discomfort on Saturday 3/21, still feeling discomfort on Sunday 3/22, employee was taken to the ER. The ER treated him for a scratched cornea, referred to an optometrist for a followup visit on Wednesday 3/25. Optometrist discovered an embedded particle, removed with a surgical drill.

Individual was walking through the Maintenance Shop. When he looked up at a piece of equipment a gust of wind blew through the shop causing some dust to get into his eye.

After returning tools to a tool box, the EE turned to walk away and felt/heard a pop in his leg. EE was not performing a task or carrying anything at this time. The EE's achilles tendon was torn. Surgery was required.

Employee got stung by a bee and had an allergic reaction. He administered an Epi-pen to himself and called his Supervisor and Dragline Maintenance Supervisor who activated the emergency response team on the minesite.

In changing out of his work shoes in the employee locker room, the employee strained a muscle in his back. At Coteau Properties request, he saw a local physician @ 10:30 am 04-23-08, who advised him to not go back to work till Monday, 04-28-08.

Security Officer was opening door to Coal Handling Fire Suppression Pump House to conduct a routine inspection. Upon grabbing the door the Security Officer was stung three times on her left index finger by wasps. The Security Officer was aware she was allergic to bee/wasp stings and started to have an allergic reaction. The Security Officer was wearing cloth gloves.

An employee was unloading a 3x3x1 section of gap grating from the bed of a pickup. A sudden gust of wind caused loose dust (dirt) to be blown into his face and eyes. Attempts to flush the dust from his left eye were unsuccessful. A doctor flushed the dust from the eye and administered an antibiotic prescription medication as a precaution.
