Employee was working on the left rear final drive of a 777 haul truck when he felt pain in his back, between the shoulder blades. The employee missed the next two scheduled shifts then returned to work.

Employee mounted and dismounted multiple pieces of equipment, around noon his ankle started hurting. He was taken to medical facilities and had a sprained ankle. The individual does not know of a specific incident that contributed to the sprain.

The operator of a 777Cat Haul Truck hit a soft spot in the haul road at an excessive speed, this resulted in a fracture to the operators right wrist, and a laceration to his chin. Note: The operator was wearing his seatbelt, also, the operator had been through the soft spot 4 times prior to the accident, and was contacted approx 1 minute prior to the accident about the soft spot.

While cutting the jacket off a piece of lectrical wire, the person cut their Left hand thumb. The cut required 3 stitches. The cut was a result of the person cutting toward the hand they were hold the wire with.

Employee was in the process of loading coal and stepped off the loading platform when he slipped and fell. Resulted in sprain/strain to the neck and upper back.

While grinding on a piece of metal a shard of metal became embedded in the welders right eye. The welder was wearing his welders hood at the time.

While tightening a hose clamp with an air impact wrench and a Snap-On 15/16 IPL30B Socket(swivel-type), the mechanic was holding the socket onto the bolt when the swivel pin broke, the socket seperated and part of the socket caught the mechanics Left hand Index finger, cutting it. This injury required Four stitches.

EE WAS UP UNDER THE 9N DOZER CUTTING ROLLER FLANGES OFF. IN DOING THIS, A PIECE OF THE ROLLER FLANGE FELL OFF AND GOT CAUGHT BETWEEN HIS WRIST AND GLOVE, BURNING HIS LEFT WRIST.

SUPERVISOR WAS WORKING ON THE TOUR BUS AND TRASH FELL UNDER SAFETY GLASSES INTO EYE.
