Employee was working near screen plant. Wind blew sand underneath safety glasses. On 05/07/2025 we were notified the Employee sought treatment and was Dx with a corneal abrasion. Employee was prescribed eye drops. Reporting within 10 days.

Showing all 6Employee was standing on the crusher platform, heating up oil, when they went to step up 3' onto the second platform. EE felt a pop in their left knee. They sought medical attention on 12/06/2024 and was ref. to PT. Reporting within 10 days.
On August 27th, employee advised EE had been hospitalized due to flu like symptoms. After multiple tests employee state EE was diagnosed with a water type pneumonia. Medical records have been requested to verify work relatedness, but they have not been received to date.
Employee was stung by wasp while preparing for plant move. While employee was being treated at Urgent Care employee lost consciousness. Employee was released a few hours later after being treated at the hospital.
EE was walking on ground at surface mine and had seizure of unknown origin. EE is still in hospital undergoing testing.
Strained back doing mechanical work on plant - twisting etc.
