Struck against stationary object

Employee went to flip the switch on the disconnect box to start plant components when they cut their finger on a sharp edge of the switch.

Caught in, under or between a moving and a stationary object

EE was performing maintenance on cone crusher replacing sheave. Sheave shifted off motor shaft. EE's left thumb got caught between impact driver handle & steel frame. Sought med. treatment same day, received stitches & removed from work. Reporting within 10 days.

Employee was walking on jobsite and stepped on a rock and rolled EE's left ankle. EE is currently diagnosed with a sprain.

Employee was lifting bucket & felt pain in right arm/shoulder. Employee sought medical treatment & was given restrictions & referred to physical therapy.

Employee was pulling a silt barrier across a wash pad & strained knee. EE sought medical treatment & was recently referred to physical therapy. Due to this recent treatment this is now MSHA reportable.

EE was cutting plastic with a utility knife, the plastic slipped and the knife cut his right index finger. EE received stitches and was taken off work for a few days. EE was wearing his gloves.

Belt on crusher stopped so employee had to shovel stones off of conveyor in order to fix conveyor. While employee was shoveling stones he felt pull in his back.
