Unk at this time: possible aneurysm/stroke due to diabetes. Not beleived to be related to work. EE began feeling poorly while operating frt end loader. Transported to hospital, died at hospital possiblly from multiple strokes. We have requested & are waiting for death certificate. Med info & reportability are unk.

WHILE SHOVELING SAND FROM UNDER THE PRELIMINARY CRUSHER EMPLOYEE WAS BITTEN ON THE LEFT GROIN BY A BROWN RECLUSE SPIDER. EMPLOYEE WASN'T SURE IT WAS A SPIDER BITE UNTIL HE WENT TO DOCTOR ON 4- 23-01.
