EE stated that while in the process of backing the haul truck to empty it into the #2 pit, the rear of the haul truck entered the pit causing the cab of the unit to overturn on the passenger's side. EE was able to self-extricate from the haul truck. EE was observed by co-workers to have a laceration on head and swollen right shoulder area. EE reported pain in both areas.

EE was driving a Company vehicle that was leading another to a job site and stopped in the road to look at a deer. EE driving the second vehicle didn't see that the front vehicle stopped and struck the back of the stopped vehicle. EE reports stiffness in upper back.

Haul truck operator misjudged distance of approaching haul truck and proceeded to enter the haul road causing a front glancing collision of two haulage trucks.
Concrete labor was filling fiber form bag with grout using the nozzle of a pump truck. During the process the concrete labor got grout liquid on legs. On 10/19 the concrete labor noticed an itch to their knees, on 10/20 the labor reported a slight redness in the itch area. On 10/24/22 the concrete labor reported the need to see a doctor for redness and a possible chemical burn.
Employee was raising the side gate of a flatbed truck at the same time a forklift was pulling away from the truck. The tire of the forklift struck the gate and the employee's hand was caught between the tire and the truck gate resulting in a laceration to the right index finger that required sutures.
Conditions were wet due to rain. Area was wet,slippery & rocky. While moving a Stacker, EE fell forward tracking the stacker with the remote. As EE fell EE dropped the remote in the mud realizing the remote toggle switches did not return to the neutral. EE moved to find remote, doing this a track of the stacker ran over EE's arch & toes on right foot causing fractures.
Area was wet, muddy and material in area was sticky.
