EE was unloading cable from the back of a man trip. The mantrip drove over a bundle of roof mesh. When trip got on top of the bundle it spun out and set the top piece flying into employee's right ankle causing it to roll over.

Showing all 15Employee was driving a Gehl loader into the section. He stopped at the phone to call into the section. He turned the Gehl off and was lifting the safety bar on the machine, when it rolled backwards and hit the rib.
WHILE CLEANING COAL SPILL, LOADER CAUGHT BUCKET ON ROADWAY LIP. THIS FORCED EMPLOYEE FORWARD IN HIS SEAT & HE CONTACTED THE DASH WITH HIS RT. KNEE. THIS CAUSED A CONTUSION/POSSIBLE FRACTURE OF THE RT. KNEE CAP DIAGNOSED ON 5/25/05. EE WAS SUSPENDED PENDING REQUIRED COMPANY POLICY TEST, RESULTING IN TERMINATION PRIOR TO ANY LOST WORK OR RESTRICTIONS.
The employee was taking a load of blue Aqua-line into the 12th East section. The load made contact with the pipe that was already in place and the pipe that was already hung, broke free of the tie wire securing it and hit the employee in the head.
EE WAS CAUGHT BETWEEN THE MACHINE AND THE RIB. OPERATOR DID NOT SEE HIM THERE.
Injured enmployee was operating a 975 Eicmo Tractor with a loaded trailer that was jack-knifed in the entry. In attempting to straighten out the units , the rib hit and a piece of coal rib struck the injured left hand cutting the small finger. Stitches were required.
The ee was attempting to install a shear pin between the relay bar and the face conveyor. Another ee pressurized the hydraulic cylinder that advances the face conveyor and the first ee's finger was pinched between the shear pin and the face conveyor.
EMPLOYEE WAS OPERATING A 975 TRACTOR AND MOVING IT TO THE SURFACE WHEN HE HIT A POT HOLE JAMMING HIS BACK.
EE WAS OPERATING A CAN SETTER UNDER A FAKE ROOF SUPPORTED WITH TRUSS BOLTS. THE CHAIN BINDER HOLDING NO BOTTLE CAUGHT THE TAIL OF THE TRUSS BOLT, AS THE TAIL CAME LOOSE IT HIT THE EE IN THE RIGHT EYE OF SAFETY GLASSES, PUSHING THEM INTO THE EYE AND SURROUNDING AREA.
A SHUTTLE CAR WAS BACKING AWAY FROM THE CONTINUOUS MINER. THE FRONT OF THE CAR HIT A ROCK THAT WAS LYING ON THE FLOOR. THE ROCK SLID INTO THE INJURED EE'S FOOT CAUSING LACERATIONS THAT REQUIRE D SUTURES.
EE WAS WORKING ALONG SIDE A BATTERY SCOOP THAT WAS PUSHING LOOSE WALL TOWARD THE FACE. HIS FEET SLIPPED OUT FROM UNDER HIM AND HE FELL ON THE MINE FLOOR. THE SCOOP RAN OVER HIS FOOT. HIS FOOT WAS X RAYED NO BONES WERE BROKEN, BUT THERE WAS A LACERATION THAT HAD TO BE CLEANED AND STTICHED.
A EMICO 975 MATERIAL HAULER IN MAIN HAULAGEWAY, HIT SOMETHING IN ROADWAY CAUSING INDIVIDUAL TO BOUNCE OUT OF SEAT HITTING HEAD AGAINST MINE ROOF. INDIVIDUAL CONTINUED TO HAVE PAIN IN NECK AND SAW DOCTOR ON 6/7/02. ON 6/11/02 SAW DOCTOR AGAIN AND WAS HELD OFF WORK FOR STEROID TREATMENT OF NECK; QUESTION OF DISC DAMAGE.
WHILE OPERATING A GEHL STEER SKID LOADER. THE INDIVIDUAL HAD EXTENDED HIS LEGS OUTSIDE THE OPERATOR'S COMPARTMENT WHILE TALKING TO OTHER WORKERS. UPON RESTARTING GEHL LOADER THE BUCKET DROPPED CATCHING FEET UNDER BUCKET LIFT FRAME. INDIVIDUAL HAD NOT PULLED LEGS BACK INTO OPERATOR COMPARTMENT. THIS CAUSED FRACTURES TO BOTH FEET.
INJ EE WAS OPERATING A SHIELD HAULER AS HE DROVETHRU THE FIREDOOR, THE VEHICLE STRUCK THE FRAME OF THE DOOR & A BLOCK FELL & STRUCK HIS HEAD/NOSE HE SUFFERED A FRACTURE TO THE NOSE Y
WHILE MOVING THE LONGWALL TAILGATE DRIVE WITH A WAGNER 25X THE INDIVIDUAL WALKED TO THE SIDE OF THE SCOOP AS IT WAS BEING TURNED AND THE ENGINE END OF THE SCOOP STRUCK HIM AND CAUSED A FRACTUR E TO HIS LEFT FEMUR.
