Another employee was moving a chute with a skid steer. The chute was chained & the skid steer hit a bump which caused the chute to shift in the rigging. The chute struck the employee's left foot. EE sought medical treatment & was diagnosed with 2 broken toes. EE was wearing steel toes.

Showing all 10Employee was walking around the plant & while doing so a rock bounced off a conveyor at a transfer point & hit left hand. They sought treatment & was diagnosed with a bruise & given a splint.
Employee was mowing with a riding mower per perpendicular to the slope. Due to the steepness the mower slip and the deck caught causing it to roll over several times landing on the employee causing bruising and strains to his lower extremities.
Frost was coming out of haul road truck hit rough spot in road. Operator jammed right hand on steering wheel. Bone fracture above pinky finger on right hand.
Apparently an intoxicated ATV operator was fatally injured near our inactive mine at Bloomfield. The Wisconsin DNR led our manager to believe that the accident occurred off mine property. We were informed on 12/23/2009 that the accident may have occurred on the haul road. The individual was not an employee.
Right wrist pinched between conveyor frame and chunk of wet material that had unexpectdly fallen from the bottom of an adjacent overhood conveyor. He went to md and received restrictions. On 5/18/09 he started light duty work within restrictions. He was working regular duty job until 5/18/09.
Employee complained of back pain & instally said it was not work related. Then on 6/12/07 He said this is work related and his back pain in caused by driving the guarry truck over the past year. *We are denying work relatedness of this claim*.
