Employee was walking towards machine when forklift driver crossed employee's path. Employee tried to block impact with forearm. Employee sustained shoulder strain; restricted duty as assigned.

Employee walked behind forklift that was stopped. The forklift driver backed up, not seeing the employee. The employee did not get out of the path of the forklift and the forklift ran over the employee's left foot. Restricted duty assigned.
Employee was removing material from pile with a skidsteer. He hit something, causing the skidsteer to stop suddenly. The sudden stop caused pain in his back; Diagnosis: Lumbar Strain - Restricted duty
Employee was driving loader and felt pain in his lower abdomen. Diagnosis: Abdominal wall strain; Restricted duty and prescribed medications assigned.
Employee was cleaning scrap away with his bucket. The bucket got caught on something causing the skid steer to stop quickly. When this happened the EE hit his head on something inside the skid steer. *Original injury- abrasion/contusion to forehead, not MSHA reportable. *3-6-07 complained of back pain from injury on 2-14-07- Restricted duty- low back pain.
The lever on the bobcat that is used for locking/unlocking the pin so that the operator can change from bucket to forks was frozen. The employee kept trying to get the lever to move and once it did, the lever pinched his finger between the lever & the carriage. Fracture & complex laceration right middle finger. Off work & sutures.
EE began to have back pains from driving the lift truck on rough terrain. Off work. Right thoracic back strain.
Employee was driving a loader and hit a big root causing his neck to twist to one side. He did not report accident or seek treatment until 1/8/07 cervical strain-light duty.

Employee was driving a forklift and felt strain in his upper back - back strain - restricted duty.
Two loaders were unloading hay bales from truck. His forks were on top of the other loader's forks. When the forks underneath were lifted, it caused the other loader to lift and then fall to the ground. Employee's abdominal wall was strained. Prescribed medication. EE did not report until 12/13/05.
The jerking from driving a loader caused pain in employee's back. Back strain - restricted duty.
Employee was driving skidsteer when he got dust in his eye. Corneal foreign body-went to specialist-given presciption eye drops.

EE SOUGHT TREATMENT FOR BACK PAIN CAUSED BY YEARS OF BEING BOUNCED & JARRED WHILE QUARRYING STONE WITH LOADER
Employee drives a loader. He started complaining of pain going from lower back down his right leg. He did not go to doctor until 7-22-04. Diagnosis siatica - recurring from old injury.
EMPLOYEE WAS STANDING BESIDE THE ROLLER - CONVEYOR STACKING STONES ON A PALLET. THE FORKLIFT DRIVER PLACED STONE ON THE ROLLERS WHICH STRUCK A STONE ALREADY ON THE ROLLERS, CAUSING IT TO FALL OFF & STRIKE EMPLOYEE'S KNEE. EMPLOYEE RECEIVED A CONTUSION & SPRAIN TO LEFT KNEE.
EMPLOYEE WAS PICKING A STONE UP OFF THE GROUND, WHEN ANOTHER STONE FELL OFF THE CONVEYER BELT AND HIT HIS LEFT ARM CAUSING A LACERATION THAT NEEDED STITCHES.
EMPLOYEE WAS USING A HAMMER TO CHISEL TO OPEN A STONE WHEN A LARGE ROCK FLEW OUT FROM UNDER A NEARBY LOADER TIRE AND STRUCK THE EMPLOYEE IN THE LOWER LEFT LEG. EMPLOYEE BROKE BOTH BONES IN LOW ER LEFT LEG.
EE STARTED COMPLAINING OF A SORE BACK APPROX. 1-2 WEEKS AGO. HE THOUGHT IT HAD TO DO WITH BACK SURGERY HE HAD A FEW YEARS AGO. HE WAS DOING STRETCHING EXERCISES AT A TRAINING WHEN IT BECAME WORSE. DR. DIAGNOSED A SEVERE LUMBAR STRAIN.
SEAT IN LOADER WAS BROKE, REACTIVATED BACK SORENESS RESULTED IN RESTRICTED DUTY. RECEIVED RESTRICTIONS 7-26.
WHILE DRIVING BOBCAT A STONE GOT IN EE S LEFT EYE.
EMPLOYEE WAS FLIPPING AND REFILLING WITH THE CRANE. ANOTHER EMPLOYEE WAS BACKING UP WITH THECLARK; HE WAS LOOKING OVER HIS RIGHT SHOULDER & TURNING TO THE RIGHT. AS THE INJURED GLANCED OVER HIS LEFT SHOULDER, THE OTHER EMPLOYEE RAN OVER HIS RIGHT FOOT. RESULT WAS BRUISED FOOT NEEDING ICE PAK.
