Employee stated that while operating a loader, the bucket struck a toe on the floor and caused elbow to be stuck between the arm rest and the machine controls, resulting in elbow pain. No medical treatment until 04/12/2021.
Employee stated EE was operating a skid steer with an extended rake in the bucket, when the rake fell out EE ran over the rake causing the skid steer to bounce.

The company could and would accomodate restricted duty. However, due to ee's suspected violation of company safety and health policy, the action required immediate suspension pending further investigation.
EE WAS CLEANING OUT A ROUND OF MATERIAL IN 3B-107 WHEN A SECTION OF THE ROOF FAILED, STRIKING THE BACK END OF THE LOADER. THE SECTION HAD JUST BEEN SCALED PRIOR TO LOADING THE MATERIAL.

CAUSING DROP AXLE TO SWING HARD, ST RIKING EE. THIS CAUSED FRACTUR TO RT. FIBULA, POSSIBLE HAIRLINE FRACTURE TO R ANKLE, LEFT LEG SOFT TISSUE CONTUSION & POSSIBLE L KNEE STRAIN.
