EE was operating a 980 FEL when pain was felt in the lower back.
On 8/18/21 employee complained of shoulder pain from driving haul truck. Employee received steroid injection and physical therapy.
Employee stated that while operating a loader, the bucket struck a toe on the floor and caused elbow to be stuck between the arm rest and the machine controls, resulting in elbow pain. No medical treatment until 04/12/2021.
Employee stated EE was operating a skid steer with an extended rake in the bucket, when the rake fell out EE ran over the rake causing the skid steer to bounce.
Employee stated was cleaning with a loader when a rock rolled around the bucket which EE ran over causing EE to strain EE's back.
Employee stated EE was driving the man-lift out of the mine when the man-lift hit a hole in the road causing the man-lift to rock and jarred EE's back. No medical treatment until physical therapy was prescribed on March 13.
Employee stated was standing along a conveyor while it was being started when a rock came off the conveyor and struck them on the hardhat.

EE was driving haul truck across haul road, ran over a washed out spot on the road, the movement in the truck caused his seat to bottom out and he hit his hip on the bar at the bottom of the seat. EE reports pain in low back and right hip.
Driving a scraper from the landfill , the back of the scraper started to swing around, the employee hit a dry spot and it turned over on its side. Laceration on right hand requiring 6 stitches, possible metacarpal fracture.
Employee states was driving a haul truck at approximately 12 noon when his front driver-side wheels dropped off in a hole. He reports this caused him to bounce up and down hard in his seat. Reports pain in lower back.
While driving an end dump truck, truck hit rough area in road, reports seat, bottomed out, causing neck pain with radiculopathy to right arm.

EMPLOYEE ALLEGES WHILE OPERATING THE 980G FEL. THE SEAT KEPT PINCHING HIS BACK. THE FAB. MECHANIC COULD NOT FIND ANYTHING WRONG WITH THE SEAT.
Employee alleges driving over rough roads at the quarry jarred his back (doctor stated this is from a prior injury which the employee has already received a settlement).
Employee alleges he used his right hand to reach over and close the wing vent on his truck, when he hit a bump or dip in the road twisting his middle finger. Employee didn't tell anyone until 3 weeks later.
EMPLOYEE HAD BEEN RUBBING HIS EYE COMPLAINING OF IRRITATION. HE WAS SENT TO THE HOSPITAL & A PIECE OF METAL WAS FOUND IN HIS RIGHT EYE. THE PIECE OF METAL WAS REMOVED, EE RECEIVED EYE DROPS & WENT BACK TO WORK.
Employee alleges boulders were dropped in his truck which bounced him around inside the cab. However, the ee told his superintendent that he was injured at home on April 27. The company could and would accomodate restricted duty. However, due to ee's suspected violation of company safety and health policy, the action required immediate suspension pending further investigation.
EE WAS CLEANING OUT A ROUND OF MATERIAL IN 3B-107 WHEN A SECTION OF THE ROOF FAILED, STRIKING THE BACK END OF THE LOADER. THE SECTION HAD JUST BEEN SCALED PRIOR TO LOADING THE MATERIAL.
EE stated he injured his back while driving on the haul road underground. He had already made 31 trips but hit a hole in the road on his 32nd trip.
EE was operating the 560 loader on the haul road when he hit a bump on the road twisting his back.

Allegedly the employee was hauling material to the crusher when he had to use both hands to turn the truck. Employee said his dr stated he had a torn rotator cuff. truck #68206
EE alleges he was loading trucks with high lift when he felt a twinge in his back.
While driving on the haul road he hit a chug hole in the road causing him to strain his neck and shoulder.

WHILE DRIVING A STOCKPILE TRUCK EE ALLEGEDLY HIT A POTHOLE IN THE QUARRY ROADWAY CAUSING A NECK STRAIN. THIS CLAIM WAS ULTIMATELY DENIED AS IT WAS DETERMINED TO BE A PRE-EXISTING CONDITION AND NOT WORK RELATED.
DRIVING END DUMP FROM PIT TO JAW WHEN EXPERIENCED EYE IRRITATION AND RUBBED EYE RESULTING IN ABRASION-RX EYEDROPS. DID NOT REALIZE THIS WAS RECORDABLE.
EMPLOYEE DUMPED HIS LOAD AND WAS DRIVING BACK TO LOAD SITE FOR ANOTHER WHEN HE STRUCK AN UNEVEN SPOT ON GROUND. THE SUDDEN MOVEMENT CAUSED EMPLOYEE TO STRIKE HIS RIGHT THIGH ON LEVER CONSOLE RESULTING IN INJURY.
EE WAS WASHING COMPANY TRUCK WHEN A TRUCK OWNED/OPERATED BY ANOTHER CO., WITH DROP AXLE DOWN, DROVE FROM PARKED POSITION AND DRIVER CUT WHEELS TO MAKE TURN. CAUSING DROP AXLE TO SWING HARD, ST RIKING EE. THIS CAUSED FRACTUR TO RT. FIBULA, POSSIBLE HAIRLINE FRACTURE TO R ANKLE, LEFT LEG SOFT TISSUE CONTUSION & POSSIBLE L KNEE STRAIN.
