Showing all 29Another EE was parking 31-099. Injured EE was already parked in 31-089 and preparing to dismount. According to the other EE, he did not see 31-089, as it was in a blind spot. 31-099 collided with 31-089 causing injured EE to sustain a blunt trauma to his ribcage.
Employee was operating loader & complained about eye irritaion. Eye was red & swollen following day at beginning of shift. He was wearing glasses w/side shields at time he complained of irritation.
Lack of clear communication while attempting to tow a broke down truck. The towing truck backed up when it should have pulled forward and caught employee between vehicles.

DARK CONDITIONS BACKING UP TO DUMP, MISJUDGED TRUCK PLACEMENT & BACKED INTO SIX FOOT HIGH BERM & FLIPPED THE TRUCK OVER. EMPLOYEE FAILED TO WEAR HIS SEAT BELT & WAS INJURED AS A RESULT.
Employee was walking underneath the belt to clean the magnet, and felt a thud and was on the ground. The yellow hopper went over the tail pulley of screen feed #2 (equip #7178) broke loose of its welds & pinned employees left knee to the ground. Had to use a loader & chain to lift the chute off.
The employee attempted to place a guard back over a moving conveyor; although, policies in place at the plant require the conveyor to be stoppped. The guard bumped into the moving conveyor and bounced up into the employee's chin. His chin was lacerated, requiring several stiches.
While operating Water Pull #7, hit bumps in road, caused pain in lower back. 911 called and transported to the Hosp.
EMPLOYEE WAS LOADING TRUCKS WITH FILL SAND. A LARGER ROCK MUST HAVE FALLEN FROM BUCKET. UPON BACKING UP, HE RAN OVER ROCK CAUSING LOADER TO PITCH, HE LOWERED BUCKET QUICKLY CAUSING SUDDEN JARR IN LOWER BACK. (STRAIN OF LOWER MUSCLE).
While using a bucket from a 980F Wheel Loader, this employee and another got into the bucket was hoisted up to reach aprox. 7 feet to change a whiper on a conveyer belt. Driver of the 980F had a foot that slipped off the brake, and the machine went forward pinning the injured employee.
Several employees were attempting to remove a 4' section of I beam frozen into ice. They used a bobcat to loosen the beam and when it broke free it caught the employee's finger between the beam and the wall.
EE WAS ASSISTING CONTRACT MECHANIC REMOVE CLUTCH HOUSING FROM EL-JAY VSI CRUSHER. MECHANIC WAS HOISTING HOUSING WITH LIFTING STRAP USING TRUCK CRANE. AS HOUSING CAME OUT EE HAD HAND ON LIFTING STRAP & THE HOUSING SHIFTED PINCHING EE'S LEFT HAND BETWEEN STRAP & SHEAVES ON CLUTCH HOUSING. INJURY WAS CONTUSION TO LEFT HAND REQUIRING MODIFIED WORK RESTRICTIONS. NO LOST TIME.
EE WAS TALKING TO SKID STEER OPERATOR WHEN THE OPERATOR STARTED TO PULL AWAY FROM THE EE. THE EE (MECHANIC) GOT HIS RIGHT WRIST CAUGHT IN THE WINDOW TO THE MACHINE CAUSING A CONTUSION ON WRIST & STRAIN OF RIGHT SHOULDER.
EMPLOYEE WAS IN HAUL TRUCK BEING LOADED BY A FRONT-END LOADER ON THE QUARRY FLOOR. A ROCK FELL FROM THE ADJACENT HIGH WALL AND ROLLED INTOT HE BACK OF THE HAUL TRUCK DRIVING THE TRUCK FORWARD. THE EMPLOYEE SUSTAINED A WHIPLASH TYPE INJURY. THE LEFT TAIL SIDE WAS DAMAGED APPOX $250 DAMAGE.
EE WAS STANDING ON A CONVEYOR APPROXIMATELY 5' OFF OF GROUND OBSERVING THE TRANSFER POINT OF THE CONVEYOR HE WAS STANDING ON AND THE DISCHARGE END OF THE CONVEYOR DUMPING ONTO IT. HE LOST HIS FOOTING & GRABBED DRIVE PULLEY OF THE OVERHEAD CONVEYOR CUTTING HIS RIGHT RING FINGER & TWISTING HIS WRIST. THE CUT TOOK 4 STITCHES.
EE WAS LOWERING COUNTERWEIGHTS ON LONG BELTS. THERE WAS STORED ENERGY IN BELT CAUSING HOIST LEVER TO SWING HITTING EE ON LEFT ARM.
BRAKEMAN RIDING ON RAILROAD CAR, CAME TO A SWITCH TO SWITCH TRACKS, THE CAR JUMPED OFF OF TRACK, THE BRAKEMAN CALLED FOR THE TRAIN TO STOP AND BEFORE THE ENGINEER COULD COMPLETELY STOP THE BRA KE, EE JUMPED CAUSING BRUISING TO LT. ELBOW AND RT. KNEE, WILL BE OFF ONE WEEK.
THE EE WAS TRAVELING ALONG ON 495 SOUTH BRINING SIXTY THE CRANE FROM WRENTUAR TO TAUTON WHEN HE HIT A HUMP ON THRU WAD AND THE INDIVIDUAL HIT HIS HEAD ON HIS CAB OF THE DRANE.
EE ALLEGES THAT HE HIT A POTHOLE ON THE ROAD ANDSUFFERED A BACK INJURY.
EMPLOYEE WAS REMOVING OVERSIZE ROCK OFF FEED TUNNEL BELT, ROCK SLIPPED FROM EE'S GRIP AND SMASHED RIGHT INDEX FINGER AGAINST OTHER MATERIAL ON CONVEYER BELT.
ATTEMPTED TO STEP UP ON RAIL CAR, MISSED STEP & FELL & WAS PULLED ALONG TRACK.
EE STARTED HIS HISFT AT 1:30 PM. HE PROCEEDED UP TWO HY-PASS WAS HEADING TOWARDS THE QUARRY. TRAVELING UP A QUARTER OF THE WAD THE 50 TON F TRUCK CAT 31-071 SUDDENLY SWORED THE BEAM PUSHING T HE HEAR DOWN THE SLOPE. THE CAB OF THE TRUCK LANDED APPROX. 20 FT OFF TWO HERE.
HAUL TRUCK DRIVER WAS HAULING WASH SAND TO STOPCKPILE. THERE WAS ADEQUATE BERMING, DRIVER FAILED TO INSPECT FRONT OF STOCKPILE WHERE IT HAD BEEN UNDER MINED. DRIVED BACKED UP TO BERM, THE STOC KPILE GAVE WAY. THE TRUCK WENT HALFWAY DOWN PILE THEN THE TRUCK FLIPPED OVER ONTO ITS ROOF. DRIVER WAS THROWN HALFWAY OUT THE DRIVER DOOR. IT BELIEVE HE WAS NOT WEARING HIS SEATBELT.
EE ALLEGES HE TWISTED HIS BACK ON THE ROUGHT TERRAIN OF THE ROAD SURFACE.
HAND PAIN.
EMPLOYEE HAD SECURED A CHAIN ON TAIL PULLEY OF A CONVEYER IN ORDER TO HOIST AND MOVE IT. HE WAS HOLDING THE CHAIN TO MAINTAIN TENSION ON THE HOOK. HE THEN MOTIONED THE LOADER OPERATOR TO BEGIN HOISTING. AS THE CHAIN BEGAN TO TIGHTEN, HE NEGLECTED TO REMOVE HIS HAND FROM THE CHAIN. HIS INDEX FINGER WAS SEVERED AT 1ST DIGIT BETWEEN CHAINAND GUARD.
