A couple weeks ago he had a crick in his neck. It went away but stayed irritated up until 08/14/12, this is when he reported to his supervisor that his neck was pain. The loader pins are worn causing the bucket to jolt his neck back and forth. The loader was taking out service on the 15th an the bucket pins replaced.

Showing all 10Operator was on the kiln firing deck observing the material inside the kiln. His helper sounded the horn on the front-end loader, where at this time the operator has instruction to go inside the control room for safety. The helper was removing hot rock from underneath the firing deck platform. Hot steam came up and burned the operator.
Process of rolling a tank to reposition. EE was told to move his chock since it was loose. There was a manway in center of the tank so had to put the forklift on the other end of the tank to releive pressue from bound end to remove that chock. At the same time the bound end had been freed the forklift was backing off tank to reposition. Tank rolled & pinned EE R Leg.

EE WAS RUNNING LATE FOR WORK HE STOPPED HIS TRUCK IN FRONT OF OFFICE. GOT OUT WHEN HIS TRUCK STARTED BACKWARD AND THE DOOR KNOCKED HIM DOWN THE LEFT FRONT WHEEL RAN OVER HIS LEFT FOOT. EE DID NOT GET TO DOCK IN FOR HIS 1130 SHIFT.

HE WAS CHANGING ROLLERS ON CONVEYOR, HIS LEFT MIDDLE FINGER WAS MASHED BETWEEN ROLLER AND BELT CAUSING BLOOD POISON UP LEFT ARM.
