There was no lifting, pushing, pulling or striking incident that caused this shoulder soreness. States "I drove over a bump in my path and it jarred me".

Showing all 8Climbing on the sandbox, above the conveyor cat walk, and standing on the conveyor. No LOTO procedures followed. Miscommunication and the conveyor was turned on while employees was climbing down. EE fell onto the catwalk below, about 2-3 feet.
Employee states he was operating a water wagon to wet the roadways. In the evening the employees back began to bother him.
Employee was giving hand signals to a front end loader operator as they were splicing a belt. The nylon choker broke and struck employee in the chest.
EMPLOYEE WAS AT THE REAR SECTION OF AN 8" PIPE THAT WAS BEING PULLED FROM A TRENCH BY A 824 JOHN DEERE LOADER. THE REAR OF THE PIPE SUDDENLY CAME UP OUT OF THE TRENCH AND STRUCK HIM IN THE CHEST.
Employee was wiring up the crusher and as the loader operator was back dragging the yard something blew into the employee's eye.
EMPLOYEE WAS UNHOOKING CHAINS FROM A CONVEYOR COVER THAT WAS BEING LOADED ON TO A FLATBED TRUCK. EMPLOYEE HAD UNHOOKED CHAINS AND WAS GOING TO UNHOOK OTHER CHAINS WHEN A BOOM POLE ATTACHED TO A FRONT END LOADEDER STRUCK HIM IN THE LEFT SHOUDER.
RIDING A 4-WEELER ACROSS THE PIT AREA AND RAN OFF A 6 TO 8 FOOT HIGHWALL. THE IMPACT CAUSED HIM TO HIT HIS FACE ON THE HANDLE BARS.HE RRECEIVED LACERATIONS TO HIS NOSE AND FACE ALSO BRUISES TO HIS BODY.
