Employee overheated while lacing conveyor belt, claimed they got dizzy and sat down beside the conveyor road, did not communicate to anyone and called 911 themselves! Employee ate and drank throughout the day. With a group of co-workers all day including the supervisor to allow breaks (water, shade, cooling) due to the extreme heat.

Employee was walking backwards from front of pickup truck along conveyor. Stepped onto the grated ground berm, twisted ankle underneath self landing with all EE's body weight. Believed ankle was just sprained and went home. Two days later (6/14/24) found out it was a nondisplaced fracture of lateral right fibula.
Laceration to left leg requiring stitches. While attempting to disconnect bucket from skid steer, the bucket slipped off the bucket moment and contacted left leg resulting in a laceration.
