Showing all 5EE was driving a Terex 3066 C Articulated Rear Dump Truck down a steep gravel haul road into a turn. The equipment's rear tandem tires began to "buck jump". EE applied brakes. Truck slid sideways and turned over on right side. EE caught arm in steering wheel and broke right humerus.

EES WERE SPREADING STONE WHEN SOME OF THE STONE BECAME STUCK IN THE TAILGATE. WHEN THE JAMMED ROCKS WERE DISLODGE, TAILGATE RELEASED AND EE'S FINGER GOT CAUGHT.
A ROCK FELL FROM BIN ABOUT 14' AND STRUCK EE IN HARD HAT- BOUNCED OFF AND STRUCK HIS NECK AND BACK.
